Key facts

Selexipag is an oral, selective prostacyclin (IP) receptor agonist. It is used for long-term treatment of pulmonary arterial hypertension (PAH) in adults.

What it is used for

  • Long-term treatment of PAH in adults with WHO functional class II–III: as combination therapy when insufficiently controlled with an endothelin receptor antagonist and/or PDE-5 inhibitor, or as monotherapy in patients who are not candidates for these therapies.

Dosing

  • Uptravi (film-coated tablets): starting dose 200 micrograms twice daily, approximately 12 hours apart; increase by 200 micrograms twice daily, usually at weekly intervals, to the highest individually tolerated dose (maximum 1600 micrograms twice daily).
  • At the start and at each up-titration step, take the first dose in the evening; if a dose is not tolerated, reduce to the previous dose level.
  • If treatment is missed for 3 days or more, restart at a lower dose; withdraw gradually while an alternative therapy is introduced.
  • Moderate CYP2C8 inhibitors (e.g. clopidogrel, deferasirox, teriflunomide): halve the total daily dose.
  • Moderate hepatic impairment (Child-Pugh B): starting dose 100 micrograms twice daily; severe hepatic impairment: do not use. Severe renal impairment: titrate with caution; not for patients on dialysis.

Contraindications

  • Hypersensitivity to the active substance or to any of the excipients.
  • Severe coronary heart disease or unstable angina; myocardial infarction within the last 6 months; decompensated cardiac failure if not under close medical supervision; severe arrhythmias.
  • Cerebrovascular events (e.g. TIA, stroke) within the last 3 months; congenital or acquired valvular defects with clinically relevant myocardial function disorders not related to pulmonary hypertension.
  • Concomitant strong CYP2C8 inhibitors (e.g. gemfibrozil).

Important warnings

  • Vasodilatory effects may lower blood pressure; caution with antihypertensives, resting hypotension, hypovolaemia or severe left ventricular outflow obstruction.
  • Hyperthyroidism has been observed; check thyroid function if symptoms occur.
  • If pulmonary oedema occurs, consider pulmonary veno-occlusive disease; discontinue if confirmed.
  • Limited experience over 75 years of age – use with caution.

Side effects

  • Most common: headache, diarrhoea, nausea and vomiting, jaw pain, myalgia, pain in extremity, arthralgia and flushing – mainly during up-titration, mostly mild to moderate.
  • Others: anaemia, hyperthyroidism, decreased appetite, weight decrease, sinus tachycardia.

Interactions

  • Strong CYP2C8 inhibitors (e.g. gemfibrozil) are contraindicated; moderate CYP2C8 inhibitors require halving the dose.
  • Pharmacokinetics not affected by warfarin; for other interactions see the full text.

Pregnancy and breastfeeding

  • Women of childbearing potential should use effective contraception; not recommended during pregnancy or without contraception.
