Key facts

Eptifibatide is an intravenous glycoprotein IIb/IIIa inhibitor that inhibits platelet aggregation. It is used in hospital together with acetylsalicylic acid and unfractionated heparin in acute coronary syndromes.

What it is used for

  • Prevention of early myocardial infarction in adults with unstable angina or non-Q-wave myocardial infarction, last chest pain within 24 hours, with ECG changes and/or elevated cardiac enzymes.
  • For use with acetylsalicylic acid and unfractionated heparin.

Dosing

  • Eptifibatide Accord solution for injection (bolus) and solution for infusion are used together: intravenous bolus of 180 microgram/kg as soon as possible after diagnosis, followed by a continuous infusion of 2 microgram/kg/min.
  • Infusion for up to 72 hours, until CABG surgery or until discharge; if PCI is performed, continue for 20-24 hours post-PCI, overall maximum 96 hours.
  • Moderate renal impairment (creatinine clearance ≥ 30 - < 50 ml/min): bolus 180 microgram/kg, then infusion of 1.0 microgram/kg/min. More severe renal impairment: contraindicated.
  • Terminate the infusion immediately if emergency cardiac surgery is needed. Hospital use only, by specialist physicians.

Contraindications

  • Hypersensitivity; gastrointestinal, gross genitourinary or other active abnormal bleeding within the previous 30 days; bleeding diathesis.
  • Stroke within 30 days or any history of haemorrhagic stroke; intracranial disease (neoplasm, arteriovenous malformation, aneurysm); major surgery or severe trauma within past 6 weeks.
  • Thrombocytopenia (< 100,000 cells/mm3); prothrombin time > 1.2 times control or INR ≥ 2.0; severe hypertension (systolic > 200 mm Hg or diastolic > 110 mm Hg on treatment).
  • Severe renal impairment (creatinine clearance < 30 ml/min) or dialysis; clinically significant hepatic impairment; concomitant another parenteral GP IIb/IIIa inhibitor.

Important warnings

  • Bleeding: observe patients carefully, especially at arterial access sites; higher risk in women, the elderly, low body weight and moderate renal impairment.
  • Immune-mediated thrombocytopenia may occur: monitor platelets before treatment, within 6 hours, and at least daily thereafter.
  • Before starting, check PT, aPTT, serum creatinine, platelet count, haemoglobin and haematocrit.
  • Contains sodium.

Side effects

  • Very common: bleeding (major and minor, e.g. femoral artery access, gastrointestinal, genitourinary, retroperitoneal, intracranial).
  • Common: cardiac arrest, ventricular fibrillation, ventricular tachycardia, congestive heart failure, atrioventricular block, atrial fibrillation, shock, hypotension, phlebitis (largely related to the underlying disease).
  • Uncommon: thrombocytopenia, cerebral ischaemia.

Interactions

  • Warfarin and dipyridamole did not appear to increase the bleeding risk.
  • Thrombolytics: limited data; bleeding risk appeared increased with streptokinase – see the full text.

Pregnancy and breastfeeding

  • Should not be used during pregnancy unless clearly necessary.
  • Interruption of breast-feeding during treatment is recommended.
