Key facts

Clopidogrel is an antiplatelet agent (P2Y12 inhibitor). It is used to prevent atherothrombotic events after myocardial infarction or stroke, in peripheral arterial disease and in acute coronary syndrome.

What it is used for

  • Secondary prevention after myocardial infarction, ischaemic stroke or in established peripheral arterial disease (adults)
  • Acute coronary syndrome (NSTE-ACS and STEMI) in combination with acetylsalicylic acid (ASA), including stent placement
  • Moderate to high-risk TIA or minor ischaemic stroke: with ASA, started within 24 hours
  • Atrial fibrillation: prevention of atherothrombotic and thromboembolic events (details in the full text)

Dosing

  • Adults and elderly: 75 mg once daily
  • NSTE-ACS: single 300 mg or 600 mg loading dose, then 75 mg daily with ASA (ASA dose not higher than 100 mg recommended)
  • STEMI, medically treated: 300 mg loading dose, then 75 mg daily; patients over 75 years start without a loading dose
  • The 300 mg tablet is intended for use as a loading dose
  • Limited experience in renal impairment and moderate hepatic disease; not to be used in children
  • Further regimens (TIA, atrial fibrillation, PCI) in the full text

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Severe hepatic impairment
  • Active pathological bleeding such as peptic ulcer or intracranial haemorrhage

Important warnings

  • Bleeding risk, especially with trauma, surgery or other medicines that increase bleeding
  • Triple antiplatelet therapy (clopidogrel + ASA + dipyridamole) not recommended in acute non-cardioembolic ischaemic stroke or TIA
  • Thrombotic thrombocytopenic purpura (TTP) reported very rarely, sometimes after short exposure
  • Reduced effect possible in CYP2C19 poor metabolisers

Side effects

  • Bleeding is the most common reaction, mostly reported in the first month of treatment
  • Other adverse reactions by frequency in the full text

Interactions

  • Oral anticoagulants: concomitant use not recommended (may increase bleeding intensity)
  • Caution with ASA, heparin, glycoprotein IIb/IIIa inhibitors, NSAIDs and SSRIs
  • Strong or moderate CYP2C19 inhibitors (e.g. omeprazole, esomeprazole, fluvoxamine, fluoxetine, fluconazole) should be discouraged

Pregnancy and breastfeeding

  • Preferably not used during pregnancy as a precaution
  • Breast-feeding should not be continued during treatment
