Key facts
Ticagrelor (Brilique) is a P2Y12 receptor inhibitor (antiplatelet) used with ASA to prevent atherothrombotic events in acute coronary syndromes or after myocardial infarction.
What it is used for
- With ASA: prevention of atherothrombotic events in adults with acute coronary syndromes
- With ASA: adults with a history of myocardial infarction and a high risk of an atherothrombotic event
Dosing
- Also a daily low maintenance dose of ASA 75-150 mg
- Acute coronary syndromes: 180 mg loading dose (two 90 mg tablets), then 90 mg twice daily for 12 months
- After PCI with increased bleeding risk, ASA may be stopped after 3 months and ticagrelor continued alone for 9 months
- History of MI of at least one year: 60 mg twice daily for extended treatment
- Missed dose: take only the next tablet at its scheduled time
- No dose adjustment in the elderly or in renal impairment
Contraindications
- Hypersensitivity to the active substance or any excipient
- Active pathological bleeding; history of intracranial haemorrhage
- Severe hepatic impairment
- Co-administration of strong CYP3A4 inhibitors (e.g. ketoconazole, clarithromycin, ritonavir, atazanavir)
Important warnings
- Increased bleeding risk; caution with bleeding propensity and with NSAIDs, oral anticoagulants or fibrinolytics
- Premature discontinuation may increase the risk of CV death, MI or stroke
- Dyspnoea, usually mild to moderate; bradyarrhythmias and ventricular pauses
- Hyperuricaemia, creatinine increase, central sleep apnoea, very rarely TTP
- High maintenance dose ASA (>300 mg) not recommended
Side effects
- Most common: bleeding and dyspnoea
- Also e.g. gastrointestinal haemorrhage, diarrhoea, nausea, dyspepsia, constipation, hypotension, vertigo, bradyarrhythmia
Interactions
- Strong CYP3A4 inhibitors: contraindicated
- Simvastatin: increased levels; doses above 40 mg daily should be weighed
- Morphine and other opioids may reduce ticagrelor efficacy
- Bradycardia-inducing medicines, anticoagulants and NSAIDs with caution
Pregnancy and breastfeeding
- Not recommended during pregnancy; women of childbearing potential should use contraception
- Breast-feeding: decide between stopping breast-feeding or therapy
