Boxed warning

  • When pregnancy is detected, discontinue perindopril erbumine as soon as possible [see Warnings and Precautions (5.4) ].

Key facts

Perindopril (as perindopril erbumine) is an ACE inhibitor. Per the US prescribing information, it is used for essential hypertension and stable coronary artery disease.

What it is used for

  • Essential hypertension, alone or with other antihypertensives, especially thiazide diuretics
  • Stable coronary artery disease to reduce the risk of cardiovascular mortality or nonfatal myocardial infarction

Dosing

  • Hypertension: start with 4 mg once a day, titrate as needed to a maximum of 16 mg per day; usual maintenance 4 mg to 8 mg in one or two doses
  • Elderly hypertensive patients: initially 4 mg daily; doses above 8 mg only with careful blood pressure monitoring
  • Stable CAD: 4 mg once daily for 2 weeks, then 8 mg once daily; over 70 years: 2 mg in the first week, 4 mg in the second week, then 8 mg once daily
  • On diuretics: consider reducing the diuretic dose before starting
  • Renal impairment: not recommended with creatinine clearance below 30 mL/min; with lesser impairment start with 2 mg/day, maximum 8 mg/day

Contraindications

  • Hypersensitivity (including angioedema) to perindopril or any other ACE inhibitor
  • Hereditary or idiopathic angioedema
  • Co-administration with aliskiren in patients with diabetes
  • Combination with a neprilysin inhibitor (e.g. sacubitril); do not give within 36 hours of switching to or from sacubitril/valsartan

Important warnings

  • Boxed warning: fetal toxicity – discontinue as soon as possible when pregnancy is detected
  • Head and neck angioedema (0.1% in US trials), possibly fatal with tongue or larynx involvement; increased risk with mTOR or neprilysin inhibitors
  • Symptomatic hypotension, especially in patients on diuretics
  • Neutropenia/agranulocytosis, impaired renal function, hyperkalemia
  • Cough

Side effects

  • Cough (12% vs 4.5% on placebo) and back pain (5.8% vs 3.1%)
  • Dizziness (dose-related), headache, asthenia
  • Hypotension, angioedema, hyperkalemia

Interactions

  • Diuretics: excessive blood pressure fall at the start of therapy
  • Potassium supplements and potassium-sparing diuretics: hyperkalemia
  • Lithium: increased lithium levels and toxicity; monitor levels frequently
  • NSAIDs including COX-2 inhibitors: worsening renal function in elderly or volume-depleted patients
  • Injectable gold: rare nitritoid reactions

Pregnancy and breastfeeding

  • Discontinue as soon as possible when pregnancy is detected; can injure and kill the developing fetus
