Boxed warning

  • When pregnancy is detected, discontinue captopril tablets as soon as possible.
  • Drugs that act directly on the renin-angiotensin system can cause injury and death to the developing fetus. See WARNINGS: Fetal Toxicity

Key facts

Captopril is an ACE inhibitor. Per the US prescribing information, it is used for hypertension, heart failure, left ventricular dysfunction after myocardial infarction and diabetic nephropathy.

What it is used for

  • Hypertension, alone or in combination, especially with thiazide-type diuretics
  • Congestive heart failure, usually with diuretics and digitalis
  • Left ventricular dysfunction (ejection fraction up to 40%) after myocardial infarction in clinically stable patients, to improve survival
  • Diabetic nephropathy (proteinuria above 500 mg/day) in type I diabetes with retinopathy

Dosing

  • Take one hour before meals; individualize the dose
  • Hypertension: start with 25 mg b.i.d. or t.i.d.; after one or two weeks may increase to 50 mg b.i.d. or t.i.d.; usual range 25 to 150 mg b.i.d. or t.i.d.
  • Heart failure: usually 25 mg t.i.d.; in vigorously diuresed, hypovolemic patients start with 6.25 or 12.5 mg t.i.d.
  • After myocardial infarction: target maintenance dose 50 mg t.i.d.; diabetic nephropathy: 25 mg t.i.d.
  • Maximum daily dose 450 mg
  • Significant renal impairment: lower initial daily dose and smaller titration steps; see the full prescribing information

Contraindications

  • Hypersensitivity to captopril or any other ACE inhibitor (e.g. angioedema with another ACE inhibitor)
  • Co-administration with aliskiren in patients with diabetes
  • Combination with a neprilysin inhibitor (e.g. sacubitril); do not give within 36 hours of switching to or from sacubitril/valsartan

Important warnings

  • Boxed warning: fetal toxicity – drugs acting on the renin-angiotensin system can injure and kill the developing fetus; discontinue as soon as possible when pregnancy is detected
  • Head and neck angioedema with possibly fatal airway obstruction; increased risk with mTOR or neprilysin inhibitors; intestinal angioedema also possible
  • Neutropenia/agranulocytosis, with higher risk in renal impairment and collagen vascular disease
  • Hypotension, particularly in patients on diuretics or with salt/volume depletion
  • Hyperkalemia, particularly with renal insufficiency, diabetes mellitus or potassium-raising drugs

Side effects

  • Rash, often with pruritus, in about 4 to 7 of 100 patients, usually in the first four weeks
  • Proteinuria in about one of 100 patients
  • Pruritus without rash in about 2 of 100 patients; flushing or pallor
  • Hypotension
  • Neutropenia/agranulocytosis, anemia, thrombocytopenia, pancytopenia

Interactions

  • Aliskiren and other renin-angiotensin system blockers; neprilysin inhibitors
  • Diuretics: enhanced blood pressure fall at the start of therapy
  • Potassium-raising drugs: hyperkalemia
  • Lithium: increased lithium levels and toxicity reported; coadminister with caution
  • mTOR inhibitors (temsirolimus, sirolimus, everolimus): increased risk of angioedema

Pregnancy and breastfeeding

  • Discontinue as soon as possible when pregnancy is detected; can injure and kill the developing fetus
