RIFLE criteria

Nephrology and urology

Also known as: RIFLE-Kriterien (akute Nierenschädigung) · RIFLE kriterleri · RIFLE criteria for acute kidney injury (ADQI 2004)

What it is for

RIFLE (Risk, Injury, Failure, Loss, End-stage kidney disease) grades acute kidney dysfunction by serum creatinine (SCr) or glomerular filtration rate (GFR) change from baseline, or by urine output (UO); the worse criterion sets the class. The change should be abrupt (1–7 days) and sustained (>24 hours).

Classification

CategoryDescription
RiskSCr ×1.5 or GFR decrease >25%; or UO <0.5 ml/kg/h for 6 hours.
InjurySCr ×2 or GFR decrease >50%; or UO <0.5 ml/kg/h for 12 hours.
FailureSCr ×3, or GFR decrease >75%, or SCr ≥4 mg/dl (≥354 µmol/l) with an acute rise of ≥0.5 mg/dl (44 µmol/l); or UO <0.3 ml/kg/h for 24 hours, or anuria for 12 hours. RIFLE-FC marks acute-on-chronic failure reached by the 4 mg/dl criterion, RIFLE-FO failure reached by the urine output criterion.
Losspersistent acute renal failure: complete loss of kidney function (need for renal replacement therapy) for more than 4 weeks.
End-stage kidney disease (E)need for dialysis for more than 3 months.
