PI-RADS v2.1

Nephrology and urology

Also known as: PI-RADS-Kategorien (Version 2.1) · PI-RADS v2.1 kategorileri · PI-RADS v2.1 assessment categories (prostate MRI)

What it is for

Five-point scale for the likelihood that a lesion on multiparametric prostate MRI is clinically significant prostate cancer (csPCa), built from T2-weighted (T2W), diffusion-weighted (DWI, with ADC) and dynamic contrast-enhanced (DCE) imaging.

Classification

CategoryDescription
PI-RADS 1very low: csPCa highly unlikely.
PI-RADS 2low: csPCa unlikely.
PI-RADS 3intermediate: csPCa equivocal.
PI-RADS 4high: csPCa likely.
PI-RADS 5very high: csPCa highly likely.
Peripheral zoneDWI score is dominant; a DWI score 3 lesion with positive DCE (early focal enhancement matching the lesion) becomes PI-RADS 4.
Transition zoneT2W score is dominant; v2.1: a T2W score 2 (atypical) nodule becomes 3 when DWI is markedly restricted (DWI score ≥4); a T2W score 3 lesion becomes 4 when DWI score is 5.
Score 5 size rule4 versus 5 on T2W or DWI: a lesion ≥1.5 cm in greatest dimension, or definite extraprostatic extension or invasive behaviour, is 5.
PI-RADS Xa component is technically inadequate or missing.
