KDIGO AKI stages

Nephrology and urology

Also known as: KDIGO-Stadien der akuten Nierenschädigung (AKI) · KDIGO akut böbrek hasarı evreleri · KDIGO definition and stages of acute kidney injury (AKI)

What it is for

Acute kidney injury (AKI) per KDIGO 2012 is any of: a rise in serum creatinine (SCr) by ≥0.3 mg/dl (≥26.5 µmol/l) within 48 hours; a rise in SCr to ≥1.5 times baseline known or presumed to have occurred within the prior 7 days; or urine volume <0.5 ml/kg/h for 6 hours. Severity is staged 1–3; the stage follows whichever criterion (creatinine or urine output) gives the higher stage.

Classification

CategoryDescription
Stage 1SCr 1.5–1.9 times baseline, or an increase of ≥0.3 mg/dl (≥26.5 µmol/l); or urine output <0.5 ml/kg/h for 6–12 hours.
Stage 2SCr 2.0–2.9 times baseline; or urine output <0.5 ml/kg/h for ≥12 hours.
Stage 3SCr 3.0 times baseline or more, or SCr rise to ≥4.0 mg/dl (≥353.6 µmol/l), or start of renal replacement therapy, or in patients under 18 years a fall in eGFR to <35 ml/min per 1.73 m²; or urine output <0.3 ml/kg/h for ≥24 hours, or anuria for ≥12 hours.
Related systemsKDIGO combines the earlier RIFLE (2004) and AKIN (2007) criteria.
