Key facts
Ranibizumab is an anti-VEGF antibody fragment given into the eye; Lucentis is used for several retinal diseases, and Susvimo is a refillable ocular implant formulation for wet age-related macular degeneration.
What it is used for
- Lucentis (adults): neovascular (wet) age-related macular degeneration, visual impairment due to diabetic macular oedema, proliferative diabetic retinopathy, visual impairment due to macular oedema secondary to retinal vein occlusion, and visual impairment due to choroidal neovascularisation.
- Lucentis 10 mg/ml solution for injection (vial) also in preterm infants: retinopathy of prematurity (zone I stage 1+, 2+, 3 or 3+; zone II stage 3+; AP-ROP).
- Susvimo (adults): wet AMD in patients who have achieved stable disease after responding to intravitreal anti-VEGF treatment.
Dosing
- Lucentis, adults: 0.5 mg (0.05 ml) as a single intravitreal injection, at least four weeks apart in the same eye; started monthly until maximum visual acuity and/or no disease activity, then intervals based on disease activity (treat-and-extend possible).
- Lucentis, preterm infants (ROP): 0.2 mg (0.02 ml) per eye; up to three injections per eye within six months if disease activity persists.
- Susvimo: 2 mg (0.02 mL) delivered via the Contivue implant, with refill-exchange every 24 weeks (approximately 6 months); supplemental 0.5 mg intravitreal ranibizumab may be given if clinically necessary. Susvimo must not be substituted for other ranibizumab formulations.
- Lucentis must be given by an ophthalmologist experienced in intravitreal injections; Susvimo implant procedures are performed by trained vitreoretinal surgeons.
Contraindications
- Hypersensitivity to the active substance or to any of the excipients.
- Active or suspected ocular or periocular infections.
- Lucentis: active severe intraocular inflammation; Susvimo: active intraocular inflammation.
Important warnings
- Intravitreal injections can cause endophthalmitis, intraocular inflammation, retinal detachment, retinal tear and traumatic cataract; aseptic technique and monitoring in the week after injection are required.
- Transient and sustained increases in intraocular pressure; pressure and optic nerve perfusion must be monitored.
- Systemic events such as non-ocular haemorrhage and arterial thromboembolic events have been reported with intravitreal VEGF inhibitors; caution after prior stroke or TIA.
- Susvimo: implant-related complications (e.g. septum dislodgement, conjunctival erosion, endophthalmitis); safety in both eyes not established.
Side effects
- Lucentis: most frequent ocular reactions include eye pain, ocular hyperaemia, increased intraocular pressure, vitritis, vitreous detachment, retinal haemorrhage, visual disturbance, floaters and conjunctival haemorrhage; non-ocular: headache, nasopharyngitis, arthralgia.
- Lucentis, serious but less frequent: endophthalmitis, blindness, retinal detachment, retinal tear, iatrogenic traumatic cataract.
- Susvimo: conjunctival haemorrhage (72.1 %), conjunctival hyperaemia (25.6 %), iritis (25 %), eye pain (20.6 %), cataract (19.1 %); serious: septum dislodgement (10.1 %), conjunctival erosion (5.2 %), endophthalmitis (2.6 %).
Interactions
- No formal interaction studies.
- Lucentis can be given at least 30 minutes after laser photocoagulation on the same day.
Pregnancy and breastfeeding
- Should not be used during pregnancy unless the benefit outweighs the potential risk; effective contraception during treatment (Susvimo: and for 12 months after the last refill-exchange).
- Lucentis: breast-feeding is not recommended.
