Key facts

Faricimab is a bispecific antibody against angiopoietin-2 and VEGF-A, given by intravitreal injection to treat wet age-related macular degeneration and macular oedema (Vabysmo).

What it is used for

  • Neovascular (wet) age-related macular degeneration (nAMD) in adults.
  • Visual impairment due to diabetic macular oedema (DME).
  • Visual impairment due to macular oedema secondary to retinal vein occlusion (branch or central RVO).

Dosing

  • nAMD: 6 mg (0.05 mL) intravitreally every 4 weeks for the first 3 doses; assess disease activity at 16 and/or 20 weeks, then every 8, 12 or 16 weeks depending on activity.
  • DME and RVO: 6 mg (0.05 mL) intravitreally every 4 weeks; 3 or more consecutive monthly injections may be needed, then individualised extension in increments of up to 4 weeks (treat-and-extend).
  • Only by a qualified physician experienced in intravitreal injections.
  • When switching from other anti-VEGF therapies, intervals are set by the physician's judgement; details: see the full text.

Contraindications

  • Hypersensitivity to the active substance or any excipient.
  • Active or suspected ocular or periocular infections.
  • Active intraocular inflammation.

Important warnings

  • Injection-related reactions such as endophthalmitis, intraocular inflammation, retinal detachment, retinal tear and traumatic cataract; use aseptic technique; patients should report pain, vision loss or redness without delay.
  • Transient intraocular pressure increases within 60 minutes; do not inject while IOP is ≥ 30 mmHg.
  • Theoretical risk of systemic events including arterial thromboembolic events.
  • Limited data on treating both eyes concurrently.

Side effects

  • Most frequent: cataract (10%), conjunctival haemorrhage (7%), vitreous detachment (4%), IOP increased (4%), vitreous floaters (4%), eye pain (3%) and retinal pigment epithelial tear (nAMD only, 3%).
  • Most serious: uveitis (0.5%), endophthalmitis (0.4%), vitritis (0.4%), retinal tear (0.2%), rhegmatogenous retinal detachment (0.1%) and traumatic cataract (< 0.1%).

Interactions

  • No interaction studies; should not be given concurrently with other systemic or ocular anti-VEGF medicines.

Pregnancy and breastfeeding

  • Women of childbearing potential should use effective contraception during treatment and for at least 3 months after the last injection; use in pregnancy only if the benefit outweighs the risk.
  • Should not be used during breast-feeding.
