Key facts

Aflibercept is an inhibitor of new blood vessel formation. As an eye injection (Eylea) it treats retinal diseases; as an infusion (Zaltrap) it is used in metastatic colorectal cancer.

What it is used for

  • Eylea (intravitreal), adults: wet age-related macular degeneration, macular oedema after retinal vein occlusion, diabetic macular oedema, myopic choroidal neovascularisation
  • Eylea 40 mg/mL: retinopathy of prematurity (ROP) in preterm infants
  • Eylea 114.3 mg/ml: wet AMD and diabetic macular oedema
  • Zaltrap (intravenous): metastatic colorectal cancer after an oxaliplatin-containing regimen, with FOLFIRI

Dosing

  • Eylea 40 mg/mL, wet AMD: 2 mg (0.05 mL) monthly for three doses, then every two months; interval may then be individualised
  • Eylea 40 mg/mL, diabetic macular oedema: 2 mg monthly for five doses, then every two months
  • Eylea 40 mg/mL, ROP: single 0.4 mg (0.01 mL) injection per eye
  • Eylea 114.3 mg/ml (AMD, DME): 8 mg (0.07 ml) monthly for 3 doses, then intervals up to every 4 months, later up to 6 months
  • Zaltrap: 4 mg/kg as a 1-hour infusion followed by FOLFIRI, every 2 weeks until progression or unacceptable toxicity
  • Further regimens (e.g. retinal vein occlusion) in the full product information

Contraindications

  • Eylea: hypersensitivity; active or suspected ocular or periocular infection; active severe intraocular inflammation
  • Zaltrap: hypersensitivity to aflibercept; not for ophthalmic/intravitreal use (hyperosmotic)

Important warnings

  • Eylea: endophthalmitis, intraocular inflammation, retinal detachment and cataract after injection; strict aseptic technique and monitoring in the week after injection
  • Eylea pre-filled syringe contains more than the dose: expel excess before injecting
  • Zaltrap: severe, sometimes fatal haemorrhage, GI perforation and fistula formation
  • Zaltrap: hypertension, arterial thromboembolic events, nephrotic syndrome; blood count before each cycle

Side effects

  • Eylea: conjunctival haemorrhage (25%), retinal haemorrhage (11%), visual acuity reduced (11%), eye pain (10%), cataract (8%), intraocular pressure increased (8%)
  • Eylea: serious injection-related eye events in less than 1 in 1,900 injections
  • Zaltrap/FOLFIRI: leucopenia, diarrhoea, neutropenia, proteinuria, stomatitis, fatigue, thrombocytopenia, hypertension

Interactions

  • Eylea: no interaction studies; combination with verteporfin PDT not studied
  • Zaltrap: no pharmacokinetic interaction with FOLFIRI found

Pregnancy and breastfeeding

  • Eylea: contraception during and for at least 3 months (Eylea 114.3 mg/ml: 4 months) after the last injection; use in pregnancy only if benefit outweighs risk
  • Zaltrap: effective contraception during and for 3 months after the last dose; inhibition of angiogenesis may harm pregnancy
