Key facts

Nebivolol (Bystolic) is a beta-blocker. Per the US label, it is used to lower blood pressure in hypertension, alone or with other antihypertensive agents.

What it is used for

  • Hypertension, as monotherapy or in combination with other antihypertensive agents

Dosing

  • Usual starting dose: 5 mg once daily, with or without food
  • If needed, increase at 2-week intervals up to 40 mg
  • Severe renal impairment (ClCr less than 30 mL/min): initial dose 2.5 mg, titrate slowly
  • Moderate hepatic impairment: initial dose 2.5 mg; not recommended in severe hepatic impairment
  • No dose adjustment for elderly patients or CYP2D6 poor metabolizers

Contraindications

  • Severe bradycardia, heart block greater than first degree, sick sinus syndrome without a pacemaker
  • Cardiogenic shock, decompensated cardiac failure
  • Severe hepatic impairment (Child-Pugh >B)
  • Hypersensitivity to any component

  • Do not stop abruptly in coronary artery disease (angina, MI, arrhythmias); taper over 1 to 2 weeks
  • Bronchospastic diseases: patients should generally not receive beta-blockers
  • May mask signs of hyperthyroidism; may aggravate peripheral vascular disease
  • Caution with anesthesia and major surgery

  • CYP2D6 inhibitors (e.g. quinidine, propafenone, fluoxetine, paroxetine): use caution
  • Do not use with other beta-blockers; monitor with reserpine or guanethidine; stop nebivolol several days before tapering clonidine
  • Digitalis glycosides and non-dihydropyridine calcium channel blockers: increased bradycardia risk

Pregnancy and breastfeeding

  • Insufficient data; beta-blockers in the third trimester may cause neonatal hypotension, bradycardia and hypoglycemia
  • Not recommended during nursing
