Metoprolol is a beta-1-selective beta-blocker. Per the US label for metoprolol succinate extended-release tablets, it is used for hypertension, angina pectoris and heart failure.

What it is used for

  • Hypertension, to lower blood pressure (adults and children 6 years and older)
  • Long-term treatment of angina pectoris
  • Heart failure, to reduce the risk of cardiovascular mortality and heart-failure hospitalization

Dosing

  • Hypertension, adults: initially 25 to 100 mg once daily, adjusted at weekly or longer intervals
  • Hypertension, children 6 years and older: initially 1 mg/kg once daily, maximum initial dose 50 mg
  • Angina: initially 100 mg once daily, increased weekly; when stopping, reduce over 1 to 2 weeks
  • Heart failure: start 25 mg once daily (NYHA Class II) or 12.5 mg (more severe heart failure); double every two weeks to the highest tolerated dose or up to 200 mg
  • Hepatic impairment: start lower and titrate gradually; no reduction needed in chronic renal failure
  • Tablets are scored and can be divided, but not crushed or chewed

Contraindications

  • Severe bradycardia, second- or third-degree heart block
  • Cardiogenic shock, decompensated heart failure
  • Sick sinus syndrome (unless a permanent pacemaker is in place)
  • Hypersensitivity to any component

Important warnings

  • Avoid abrupt cessation: angina exacerbation and myocardial infarction may occur; reduce over 1 to 2 weeks
  • Worsening heart failure during up-titration
  • Bronchospastic disease: generally avoid beta-blockers; if used, lowest possible dose
  • Bradycardia, including sinus pause, heart block and cardiac arrest
  • Pheochromocytoma: only with an alpha-blocker already initiated

Side effects

  • Hypertension and angina: mostly mild and transient; most common (>2%) are tiredness, dizziness, depression, diarrhea, shortness of breath, bradycardia and rash
  • Serious: worsening angina, heart failure or AV block, myocardial infarction

Interactions

  • Catecholamine depleting drugs (e.g., reserpine, MAO inhibitors): additive effect, hypotension or marked bradycardia
  • Strong CYP2D6 inhibitors (quinidine, fluoxetine, paroxetine, propafenone) double metoprolol concentrations
  • Digitalis, clonidine, diltiazem and verapamil: increased risk of bradycardia
  • Clonidine: withdraw the beta-blocker several days before gradual clonidine withdrawal

Pregnancy and breastfeeding

  • Pregnancy: observational data have not shown a drug-associated risk of major birth defects; inconsistent reports of growth restriction, preterm birth and perinatal mortality with beta-blockers
  • Lactation: metoprolol is present in human milk in small amounts; monitor the infant for bradycardia
