Boxed warning

  • Fluoroquinolones, including moxifloxacin hydrochloride, have been associated with disabling and potentially irreversible serious adverse reactions that have occurred together [See warnings and Precautions (5.1)], including: Tendinitis and tendon rupture [See warnings and Precautions (5.2)] Peripheral Neuropathy [See warnings and Precautions (5.3) ] Central nervous system effects [See warnings and Precautions (5.4 )]

Discontinue moxifloxacin hydrochloride immediately and avoid the use of fluoroquinolones, including moxifloxacin hydrochloride, in patients who experience any of these serious adverse reactions [See warnings and Precautions (5.1)]

  • Fluoroquinolones, including moxifloxacin hydrochloride, may exacerbate muscle weakness in patients with myasthenia gravis. Avoid moxifloxacin hydrochloride in patients with known history of myasthenia gravis [ See warnings and Precautions (5.5) ]
  • Because fluoroquinolones, including moxifloxacin hydrochloride, have been associated with serious adverse reactions [See warnings and Precautions (5.1 - 5.13 )], reserve moxifloxacin hydrochloride for use in patients who have no alternative treatment options for the following indications: Acute bacterial sinusitis [See Indications and Usage (1.6 )] Acute bacterial exacerbation of chronic bronchitis [See Indications and Usage (1.7)]

Key facts

Moxifloxacin is, per the US prescribing information, a fluoroquinolone antibiotic for bacterial infections in adults, including pneumonia, skin, intra-abdominal and respiratory infections, and plague.

What it is used for

  • Community acquired pneumonia, including multi-drug resistant Streptococcus pneumoniae.
  • Uncomplicated and complicated skin and skin structure infections; complicated intra-abdominal infections.
  • Plague (treatment and prophylaxis).
  • Acute bacterial sinusitis and acute bacterial exacerbation of chronic bronchitis only when there are no alternative treatment options.

Dosing

  • 400 mg once every 24 hours; duration depends on the infection: e.g. pneumonia 7 to 14 days, complicated skin infections 7 to 21 days, intra-abdominal infections 5 to 14 days, sinusitis 10 days, bronchitis exacerbation 5 days.
  • Take at least 4 hours before or 8 hours after products containing magnesium, aluminum, iron or zinc (e.g. antacids, sucralfate, multivitamins).
  • With or without food; drink fluids liberally.
  • No dose adjustment in renal impairment (including dialysis) or hepatic impairment; no adjustment when switching from intravenous to oral.

Contraindications

  • History of hypersensitivity to moxifloxacin or any other quinolone.

Important warnings

  • Boxed warning: disabling and potentially irreversible serious reactions such as tendinitis and tendon rupture, peripheral neuropathy and CNS effects – stop immediately; exacerbation of myasthenia gravis; for sinusitis and bronchitis exacerbation use only without alternatives.
  • Tendon risk is higher over 60 years of age, with corticosteroids and after kidney, heart or lung transplants.
  • QT prolongation: avoid with known QT prolongation, uncorrected hypokalemia or hypomagnesemia and Class IA or III antiarrhythmics.
  • Serious hypersensitivity reactions, Clostridium difficile-associated diarrhea, blood glucose disturbances and photosensitivity.

Side effects

  • Most common: nausea, diarrhea, headache and dizziness.
  • Postmarketing include ventricular tachyarrhythmias, hearing impairment, agranulocytosis, pancytopenia, and severe skin, liver and kidney reactions.

Interactions

  • Antacids, sucralfate, multivitamins and other multivalent cations substantially reduce absorption; separate the doses.
  • Warfarin: enhanced anticoagulant effect possible; monitor INR closely.
  • Antidiabetic agents: hypo- and hyperglycemia reported; monitor blood glucose.
  • Class IA (e.g. quinidine) and Class III (e.g. amiodarone, sotalol) antiarrhythmics: risk of QT prolongation.

Pregnancy and breastfeeding

  • No human data establish a drug-associated risk; based on animal studies, moxifloxacin may cause fetal harm.
  • It is not known whether moxifloxacin is present in human milk.
