Key facts
Canagliflozin (Invokana) is an SGLT2 inhibitor that increases urinary glucose excretion and is used to treat type 2 diabetes.
What it is used for
- Insufficiently controlled type 2 diabetes in adults and children aged 10 years and older, as an adjunct to diet and exercise: as monotherapy when metformin is inappropriate, or in addition to other diabetes medicines.
Dosing
- Starting dose 100 mg once daily; in patients tolerating it, with eGFR ≥60 mL/min/1.73 m2 and needing tighter control, the dose can be increased to 300 mg once daily.
- eGFR 30 to < 60: 100 mg; eGFR < 30: do not initiate, but patients already taking it (with albuminuria) continue 100 mg until dialysis or renal transplantation.
- Diabetic kidney disease as add-on to standard of care: 100 mg once daily.
- Care when increasing the dose in patients ≥75 years, with known cardiovascular disease, and in children weighing < 50 kg; correct volume depletion before starting.
- With insulin or sulphonylureas, a lower dose of these may be needed. Not recommended in severe hepatic impairment.
Contraindications
- Hypersensitivity to the active substance or any excipient.
Important warnings
- Diabetic ketoacidosis may occur; not for use in type 1 diabetes.
- Increased risk of lower limb amputation (mainly toe and midfoot) in patients with cardiovascular disease or risk factors; preventive foot care is important and stopping may be considered with skin ulcers or gangrene.
- Volume depletion with dizziness and hypotension, particularly at eGFR < 60 and with 300 mg; renal function should be checked before starting and at least annually.
- Necrotising fasciitis of the perineum (Fournier's gangrene): seek urgent care for genital or perineal pain, redness or swelling with fever.
- Haematocrit may increase.
Side effects
- Very common: vulvovaginal candidiasis, hypoglycaemia in combination with insulin or sulphonylurea.
- Common: urinary tract infection, balanitis, polyuria, thirst, constipation, nausea, dyslipidaemia.
- Uncommon: dehydration, postural dizziness, syncope, hypotension, lower limb amputations; rare: diabetic ketoacidosis, anaphylactic reaction.
Interactions
- Diuretics: additive effect with risk of dehydration and hypotension.
- Enzyme inducers (e.g. rifampicin, St John's wort, phenytoin, carbamazepine) may decrease canagliflozin exposure.
- Digoxin: AUC increased by 20%; patients on digoxin should be monitored.
Pregnancy and breastfeeding
- Should not be used during pregnancy; discontinue if pregnancy is detected.
- Should not be used during breast-feeding.
