Key facts
Empagliflozin is a sodium-glucose co-transporter 2 (SGLT2) inhibitor. It is used in type 2 diabetes mellitus, symptomatic chronic heart failure and chronic kidney disease.
What it is used for
- Insufficiently controlled type 2 diabetes in adults and children aged 10 years and above, as an adjunct to diet and exercise: monotherapy when metformin is not tolerated, or added to other diabetes medicines
- Symptomatic chronic heart failure in adults
- Chronic kidney disease in adults
Dosing
- Type 2 diabetes: starting dose 10 mg once daily; if tolerated, eGFR ≥ 60 mL/min/1.73 m² and tighter control needed, increase to 25 mg (maximum)
- Heart failure and chronic kidney disease: 10 mg once daily
- eGFR < 60 mL/min/1.73 m²: daily dose 10 mg; initiation not recommended at eGFR < 20 mL/min/1.73 m²
- Type 2 diabetes: glucose-lowering efficacy reduced at eGFR < 45 mL/min/1.73 m²
- With a sulphonylurea or insulin: consider a lower dose of these
- Severe hepatic impairment: not recommended; patients 75 years and older have increased risk of volume depletion
Contraindications
- Hypersensitivity to the active substance or any excipient
Important warnings
- Should not be used in type 1 diabetes
- Ketoacidosis, including fatal cases, sometimes with only moderately raised glucose; discontinue immediately if suspected
- Necrotising fasciitis of the perineum (Fournier's gangrene), rare but life-threatening
- Volume depletion, particularly in elderly patients and with diuretics
- Lower limb amputations (primarily toe) seen with another SGLT2 inhibitor; counsel on preventive foot care
- Hepatic injury reported in trials; causal relationship not established
Side effects
- Most frequent in type 2 diabetes: hypoglycaemia when used with a sulphonylurea or insulin
- Heart failure studies: volume depletion most frequent
- Genital infections (e.g. vaginal moniliasis, balanitis) and urinary tract infections
- Others: thirst, constipation, pruritus, rash; rarely ketoacidosis and Fournier's gangrene
Interactions
- Thiazide and loop diuretics: risk of dehydration and hypotension
- Insulin and sulphonylureas: increased hypoglycaemia risk; lower dose may be required
- Lithium: levels may decrease; monitor serum lithium more often
Pregnancy and breastfeeding
- As a precaution, preferable to avoid during pregnancy
- Should not be used during breast-feeding
