Key facts
Apixaban is an oral direct factor Xa inhibitor (anticoagulant). It is used to prevent and treat venous thromboembolism and to prevent stroke in non-valvular atrial fibrillation.
What it is used for
- Prevention of venous thromboembolic events (VTE) in adults after elective hip or knee replacement surgery (2.5 mg tablets)
- Prevention of stroke and systemic embolism in adults with non-valvular atrial fibrillation (NVAF) and one or more risk factors
- Treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE), and prevention of recurrent DVT and PE in adults
- Paediatric patients from 28 days to less than 18 years: treatment of VTE and prevention of recurrent VTE
Dosing
- VTE prevention after hip/knee replacement: 2.5 mg twice daily, first dose 12 to 24 hours after surgery; 32 to 38 days (hip) or 10 to 14 days (knee)
- NVAF: 5 mg twice daily, continued long-term
- NVAF dose reduction to 2.5 mg twice daily with at least two of: age ≥ 80 years, body weight ≤ 60 kg, serum creatinine ≥ 1.5 mg/dL
- Acute DVT/PE: 10 mg twice daily for the first 7 days, then 5 mg twice daily
- Prevention of recurrent DVT/PE after 6 months of treatment: 2.5 mg twice daily
- Children: weight-tiered dosing after at least 5 days of parenteral anticoagulation; granule forms for patients < 35 kg; details in the full text
Contraindications
- Hypersensitivity to the active substance or any excipient
- Active clinically significant bleeding
- Hepatic disease associated with coagulopathy and clinically relevant bleeding risk
- Lesions or conditions that are a significant risk factor for major bleeding (e.g. current gastrointestinal ulceration, recent intracranial haemorrhage)
- Concomitant treatment with any other anticoagulant
Important warnings
- Haemorrhage risk: observe carefully for signs of bleeding; discontinue if severe haemorrhage occurs
- A specific reversal agent (andexanet alfa) is available for adults; its safety and efficacy are not established in children
- Concomitant antiplatelet agents increase the risk of bleeding
- No routine exposure monitoring is required; a calibrated anti-factor Xa assay may help in exceptional situations such as overdose or emergency surgery
Side effects
- Common: haemorrhage, contusion, epistaxis and haematoma
- Major bleeding, including gastrointestinal and intraocular bleeding, has been reported
Interactions
- Strong inhibitors of both CYP3A4 and P-gp (e.g. ketoconazole, itraconazole, voriconazole, posaconazole, ritonavir): concomitant systemic use not recommended
- Ketoconazole led to a 2-fold increase in mean apixaban AUC
- Other anticoagulants are contraindicated; antiplatelet agents increase bleeding risk
Pregnancy and breastfeeding
- Pregnancy: no human data; as a precaution, use during pregnancy should preferably be avoided
- Breast-feeding: excretion in human milk unknown (shown in animals); a decision is needed to stop breast-feeding or stop/abstain from therapy
