Key facts
Alogliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor. It is used in adults with type 2 diabetes in combination with other glucose-lowering medicines, including insulin.
What it is used for
- Adults aged 18 years and older with type 2 diabetes mellitus, to improve glycaemic control in combination with other glucose-lowering medicines including insulin, when these together with diet and exercise do not provide adequate control (Vipidia).
Dosing
- Vipidia is available as 25 mg, 12.5 mg and 6.25 mg film-coated tablets.
- Recommended dose: 25 mg once daily as add-on to metformin, a thiazolidinedione, a sulphonylurea or insulin, or as triple therapy with metformin and a thiazolidinedione or insulin.
- With a sulphonylurea or insulin, a lower dose of these may be considered to reduce the risk of hypoglycaemia.
- Moderate renal impairment (CrCl ≥ 30 to ≤ 50 mL/min): 12.5 mg once daily; severe renal impairment (CrCl < 30 mL/min) or dialysis: 6.25 mg once daily. Assess renal function before starting and periodically thereafter.
- Mild to moderate hepatic impairment: no adjustment; not recommended in severe hepatic impairment (Child-Pugh > 9).
- Elderly: conservative dosing because of possible reduced renal function. Not to be used in children and adolescents.
Contraindications
- Hypersensitivity to the active substance or any excipient.
- History of a serious hypersensitivity reaction (e.g. anaphylaxis, anaphylactic shock, angioedema) to any DPP-4 inhibitor.
Important warnings
- Not for type 1 diabetes or diabetic ketoacidosis; not a substitute for insulin.
- Increased risk of hypoglycaemia with a sulphonylurea, insulin or thiazolidinedione plus metformin.
- Acute pancreatitis: discontinue if suspected and do not restart if confirmed; caution with a history of pancreatitis.
- Reports of hepatic dysfunction including hepatic failure: check liver tests if symptoms occur.
- Hypersensitivity reactions and bullous pemphigoid: discontinue if bullous pemphigoid is suspected.
- Limited experience in heart failure NYHA class III and IV; caution is warranted.
Side effects
- Common: upper respiratory tract infections, nasopharyngitis, hypoglycaemia, headache, abdominal pain, gastro-oesophageal reflux disease, diarrhoea, pruritus, rash.
- Frequency not known: hypersensitivity, acute pancreatitis, hepatic dysfunction including hepatic failure, Stevens-Johnson syndrome, erythema multiforme, angioedema, urticaria, bullous pemphigoid, interstitial nephritis.
Interactions
- Alogliptin is hardly metabolised by CYP450; interactions with CYP inhibitors are not expected.
- Gemfibrozil, fluconazole, ketoconazole, ciclosporin, digoxin, metformin, pioglitazone and atorvastatin had no clinically relevant effect on alogliptin pharmacokinetics.
Pregnancy and breastfeeding
- No data in pregnant women; as a precaution, avoid use during pregnancy.
- Breast-feeding: a risk to the suckling child cannot be excluded; weigh breast-feeding against therapy.
