Classification

CategoryDescription
Grade 1no symptoms or mild headache, slight neck stiffness.
Grade 2moderate to severe headache, neck stiffness; no neurological deficit other than a cranial nerve palsy.
Grade 3drowsiness or confusion; mild focal deficit.
Grade 4stupor; moderate to severe hemiparesis; possibly early decerebrate posturing and autonomic disturbance.

Modifierserious systemic disease (e.g. hypertension, diabetes, severe atherosclerosis, chronic lung disease) or severe vasospasm on angiography moves the patient one grade worse; this rule is applied inconsistently in practice.
