Fisher scale

Neurology

Also known as: Fisher grade · Fisher-Skala (original) · Fisher-Klassifikation · Fisher skalası · Fisher scale (original, CT blood in subarachnoid haemorrhage)

What it is for

Fisher scale (original): amount and distribution of blood on the first CT after aneurysmal subarachnoid haemorrhage (SAH), designed to estimate the likelihood of cerebral vasospasm.

Classification

CategoryDescription
Grade 1no blood detected.
Grade 2diffuse thin layer of subarachnoid blood (vertical layers under 1 mm), no clots.
Grade 3localised clot and/or vertical layers of blood 1 mm or thicker.
Grade 4intracerebral or intraventricular blood, with diffuse or no subarachnoid blood.
Notein the original scale grade 3, not grade 4, carried the highest vasospasm association; the modified Fisher scale was developed partly for this reason. State which scale is meant.
