Key facts

Voriconazole is a broad-spectrum triazole antifungal (Vfend). It is used in adults and children aged 2 years and above to treat and prevent serious invasive fungal infections.

What it is used for

  • Invasive aspergillosis
  • Candidaemia in non-neutropenic patients; fluconazole-resistant serious invasive Candida infections (including C. krusei)
  • Serious fungal infections caused by Scedosporium spp. and Fusarium spp.
  • Prophylaxis of invasive fungal infections in high-risk allogeneic HSCT recipients

Dosing

  • Adults, intravenous: loading 6 mg/kg every 12 hours (first 24 hours), then 4 mg/kg twice daily
  • Adults, oral 40 kg and above: loading 400 mg every 12 hours, then 200 mg twice daily (under 40 kg: 200 mg and 100 mg)
  • If response is inadequate, oral up to 300 mg twice daily (under 40 kg 150 mg)
  • Children 2 to <12 years: IV loading 9 mg/kg, maintenance 8 mg/kg; oral 9 mg/kg twice daily (maximum 350 mg)
  • Correct electrolyte disturbances before and during therapy; exposure beyond 180 days requires benefit-risk assessment

Contraindications

  • Hypersensitivity to the active substance or any of the excipients
  • CYP3A4 substrates with risk of serious reactions, e.g., terfenadine, astemizole, cisapride, pimozide, quinidine, ivabradine, ergot alkaloids, sirolimus
  • Strong CYP3A4 inducers that significantly reduce voriconazole levels (list in the full text)

Important warnings

  • QTc prolongation and rare torsades de pointes; caution in proarrhythmic conditions
  • Hepatic toxicity; monitor liver function
  • Caution in hypersensitivity to other azoles
  • Intravenous formulation no longer than 6 months

Side effects

  • Most common: visual impairment, pyrexia, rash, vomiting, nausea, diarrhoea, headache
  • Peripheral oedema, abnormal liver function tests, respiratory distress, abdominal pain
  • Generally mild to moderate

Interactions

  • Metabolised by and inhibits CYP2C19, CYP2C9 and CYP3A4; a strong CYP3A4 inhibitor
  • Can markedly raise levels of CYP3A4 substrates; inducers lower voriconazole levels
  • With efavirenz: increase voriconazole maintenance dose and reduce efavirenz dose (see full text)

Pregnancy and breastfeeding

  • Not used in pregnancy unless benefit to the mother clearly outweighs the risk to the foetus; effective contraception
  • Breast-feeding must be stopped on initiation of treatment
