Key facts

Semaglutide is a GLP-1 receptor agonist. Depending on the product it is used for type 2 diabetes (Ozempic, Rybelsus), weight management (Wegovy) and MASH with liver fibrosis (Kayshild).

What it is used for

  • Ozempic (injection) and Rybelsus (tablets): insufficiently controlled type 2 diabetes in adults, adjunct to diet and exercise, alone or in combination
  • Wegovy: weight management in adults with BMI ≥ 30 kg/m2, or ≥ 27 kg/m2 with at least one weight-related comorbidity
  • Wegovy injection: also adolescents aged 12 years and above with obesity and body weight above 60 kg
  • Kayshild: non-cirrhotic MASH with moderate to advanced liver fibrosis (F2 to F3) in adults

Dosing

  • Ozempic: start 0.25 mg once weekly, after 4 weeks 0.5 mg; if needed stepwise to 1 mg or 2 mg (maximum 2 mg weekly)
  • Wegovy injection and Kayshild: escalation over 16 weeks from 0.25 mg to a maintenance dose of 2.4 mg once weekly; Wegovy may go to 7.2 mg in adults with BMI ≥ 30 kg/m2
  • Rybelsus 1.5/4/9 mg: start 1.5 mg daily for one month, then 4 mg; Rybelsus 3/7/14 mg: start 3 mg, then 7 mg; maximum 50 mg daily
  • Wegovy tablets: start 1.5 mg daily, escalating via 4 mg and 9 mg to 25 mg daily
  • Tablets on an empty stomach with a sip of water; wait at least 30 minutes before eating, drinking or other oral medicines
  • With a sulfonylurea or insulin: consider reducing their dose (hypoglycaemia risk)

Contraindications

  • Hypersensitivity to the active substance or excipients

Important warnings

  • Acute pancreatitis; discontinue if suspected and do not restart if confirmed
  • Pulmonary aspiration risk with general anaesthesia or deep sedation due to delayed gastric emptying
  • Gastrointestinal adverse reactions may cause dehydration and, rarely, deterioration of renal function
  • Type 2 diabetes: diabetic retinopathy complications; not a substitute for insulin, not for diabetic ketoacidosis

Side effects

  • Very common: nausea and diarrhoea; common vomiting, constipation, abdominal pain
  • Gastrointestinal reactions are mostly mild or moderate, mainly during dose escalation
  • Fatigue; hypoglycaemia when combined with insulin or a sulfonylurea

Interactions

  • Delayed gastric emptying may affect absorption of oral medicines; caution with medicines that need rapid absorption
  • Rybelsus: increased thyroxine exposure, consider monitoring thyroid parameters
  • Insulin and sulfonylureas: increased hypoglycaemia risk

Pregnancy and breastfeeding

  • Not to be used in pregnancy; discontinue at least 2 months before a planned pregnancy; contraception recommended
  • Should not be used during breast-feeding (all products)
