Key facts
Prednisolone is a synthetic glucocorticoid. Per the US label, the oral solution is used for many allergic, dermatologic, endocrine, rheumatic, renal and other inflammatory conditions.
What it is used for
- Severe allergic conditions (e.g. asthma, allergic rhinitis, atopic dermatitis) in adults and children
- Dermatologic diseases such as pemphigus and severe erythema multiforme (Stevens-Johnson syndrome)
- Nephrotic syndrome, to induce diuresis or remission of proteinuria
- Adrenocortical insufficiency and other endocrine disorders
- Acute exacerbations of multiple sclerosis; uveitis, sarcoidosis and further indications in the full text
Dosing
- Initial dosage 5 to 60 mg prednisolone base per day (5 mL to 60 mL), depending on the disease
- Dosage is individualised by disease and response; after a favourable response, decrease gradually to the lowest effective maintenance dose
- If there is no satisfactory response after a reasonable period, discontinue and use other therapy
- Higher doses may be needed during stress; withdraw gradually after long-term therapy
Contraindications
- Systemic fungal infections
- Hypersensitivity to the drug or any component
Important warnings
- Unusual stress requires increased dosage of rapidly acting corticosteroids
- HPA axis suppression with possible glucocorticoid insufficiency after withdrawal
- Elevated blood pressure, salt and water retention, potassium loss, increased calcium excretion
- Live or live attenuated vaccines contraindicated with immunosuppressive doses; increased susceptibility to infection
Side effects
- Endocrine: cushingoid state, decreased carbohydrate tolerance, growth suppression in children, menstrual irregularities
- Skin: impaired wound healing, thin fragile skin, petechiae
- Fluid/electrolytes: fluid retention, hypertension, hypokalaemic alkalosis, heart failure in susceptible patients
Interactions
- Enzyme inducers (barbiturates, phenytoin, ephedrine, rifampin) enhance metabolism; dose increase may be needed
- Cyclosporin: increased activity of both drugs may occur

- Teratogenic in animal studies; no adequate studies in pregnant women
- Appears in human milk; caution in nursing women
