Key facts

Pravastatin is an HMG-CoA reductase inhibitor (statin). Per the US prescribing information, it is used to lower LDL cholesterol and to reduce the risk of cardiovascular events in adults with or without coronary heart disease, and in children with familial hypercholesterolemia.

What it is used for

  • Reduction of the risk of myocardial infarction, revascularization and cardiovascular death in adults with elevated LDL-C without evident coronary heart disease
  • Reduction of coronary death, myocardial infarction, revascularization, stroke/TIA and slowing of coronary atherosclerosis in adults with evident CHD
  • Adjunct to diet in primary hyperlipidemia, primary dysbetalipoproteinemia and hypertriglyceridemia in adults
  • Heterozygous familial hypercholesterolemia in children aged 8 years and older

Dosing

  • Adults: 40 mg to 80 mg once daily at any time, with or without food; check LDL-C as early as 4 weeks after starting
  • Children 8 to 13 years: 20 mg once daily; 14 to 18 years: starting dose 40 mg once daily
  • Severe renal impairment: start 10 mg once daily, maximum 40 mg once daily
  • Cyclosporine: start 10 mg, maximum 20 mg once daily; clarithromycin or erythromycin: maximum 40 mg once daily
  • Bile acid sequestrants: take pravastatin at least 1 hour before or 4 hours after

Contraindications

  • Acute liver failure or decompensated cirrhosis
  • Hypersensitivity to pravastatin or any excipient

Important warnings

  • Myopathy and rhabdomyolysis, rarely fatal; risk factors include age 65 years or greater, uncontrolled hypothyroidism, renal impairment, certain drugs and higher doses
  • Immune-mediated necrotizing myopathy (rare)
  • Hepatic dysfunction: transaminase increases; stop if serious liver injury or jaundice occurs
  • Increases in HbA1c and fasting serum glucose

Side effects

  • Reported in ≥ 2% and more often than placebo: nausea/vomiting (7.4%), diarrhea (6.7%), headache (6.3%), upper respiratory infection, angina pectoris and rash (4.5% each)
  • Also: CK increase, dizziness, ALT increase, chest pain, cough, myalgia
  • Serious: rhabdomyolysis, liver injury

Interactions

  • Gemfibrozil: avoid
  • Cyclosporine, clarithromycin, erythromycin: dose limits
  • Niacin, fibrates and colchicine: increased myopathy risk
  • Bile acid sequestrants: separate dosing

Pregnancy and breastfeeding

  • May cause fetal harm; discontinue when pregnancy is recognized, or consider the individual patient's needs
  • Present in human milk
