What it is used for

  • Slowing progression of coronary atherosclerosis in coronary heart disease

  • Adolescents 10 to 17 years of age with heterozygous familial hypercholesterolemia (girls at least one year post-menarche)

Dosing

  • In addition to a cholesterol-lowering diet; give with meals
  • Adults: start with 20 mg once a day with the evening meal; range 10 to 80 mg/day in one or two doses; maximum 80 mg/day; adjust at intervals of 4 weeks or more
  • With danazol, diltiazem, dronedarone or verapamil: start with 10 mg, not exceeding 20 mg/day
  • With amiodarone: not exceeding 40 mg/day
  • Adolescents with heterozygous familial hypercholesterolemia: 10 to 40 mg/day, maximum 40 mg/day
  • Severe renal insufficiency (creatinine clearance below 30 mL/min): doses above 20 mg/day only after careful consideration

Contraindications

  • Hypersensitivity to any component

  • Pregnancy and lactation

Important warnings

  • Myopathy and rhabdomyolysis, dose-related, with rare fatalities; risk increased by interacting drugs

  • Avoid grapefruit juice

Side effects

  • Flatulence, constipation, diarrhea, abdominal pain, nausea
  • Myalgia, muscle cramps, headache, rash, asthenia
  • CK elevations of at least twice normal in about 11% of patients; transaminase increases

  • Gemfibrozil and cyclosporine: avoid the combination
  • Other fibrates or niacin (≥1 g/day): use caution
