Boxed warning

Antidepressants increased the risk of suicidal thoughts and behaviors in pediatric and young adult patients in short-term studies. Closely monitor all antidepressant-treated patients for clinical worsening, and for emergence of suicidal thoughts and behaviors [see Warnings and Precautions (5.1)]. Paroxetine is not approved for use in pediatric patients [see Use in Specific Populations (8.4)].

Paroxetine is a selective serotonin reuptake inhibitor (SSRI). Per the US label, it is used in adults for depression and several anxiety and obsessive-compulsive disorders.

What it is used for

  • Major depressive disorder (MDD)
  • Obsessive compulsive disorder (OCD) and panic disorder
  • Social anxiety disorder and generalized anxiety disorder
  • Posttraumatic stress disorder (PTSD)
  • Not approved for use in pediatric patients

Dosing

  • Once daily in the morning, with or without food
  • MDD and PTSD: start 20 mg, maximum 50 mg; OCD: start 20 mg, maximum 60 mg; panic disorder: start 10 mg, maximum 60 mg
  • Social anxiety disorder and GAD: 20 mg daily
  • Increase in 10 mg per day increments at intervals of at least 1 week
  • Elderly, severe renal or hepatic impairment: start 10 mg, not exceeding 40 mg/day
  • Taper gradually; at least 14 days between paroxetine and MAOIs

Contraindications

  • Taking MAOIs (including linezolid and intravenous methylene blue) or within 14 days of stopping them
  • Concomitant thioridazine or pimozide (QT prolongation)
  • Known hypersensitivity to paroxetine or inactive ingredients

Important warnings

  • Boxed warning: increased risk of suicidal thoughts and behaviors in pediatric and young adult patients
  • Serotonin syndrome; increased risk of bleeding
  • Screen for bipolar disorder before starting (mania/hypomania may be activated)
  • Discontinuation syndrome, seizures, angle-closure glaucoma, hyponatremia, bone fracture, sexual dysfunction

Side effects

  • Common: nausea, somnolence, dizziness, insomnia, tremor, sweating, asthenia, decreased appetite
  • Dry mouth, constipation
  • Sexual dysfunction such as ejaculatory disturbance and decreased libido

Interactions

  • MAOIs and other serotonergic drugs: serotonin syndrome
  • Pimozide and thioridazine: QTc prolongation and ventricular arrhythmias
  • Tamoxifen: efficacy may be reduced via CYP2D6 inhibition

Pregnancy and breastfeeding

  • First-trimester exposure is associated with an increased rate of cardiovascular malformations; consider other options when planning pregnancy
  • Present in human milk; agitation and poor feeding reported in breastfed infants
