Boxed warning
- When pregnancy is detected, discontinue olmesartan medoxomil tablets as soon as possible (5.1, 8.1).
- Drugs that act directly on the renin-angiotensin system can cause injury and death to the developing fetus (5.1, 8.1).

What it is used for
- Hypertension in adults and children six years of age and older, alone or with other antihypertensives
Dosing
- Adults: usually 20 mg once daily; may be increased to 40 mg after 2 weeks; higher doses add no effect
- Possible volume depletion (e.g. on diuretics): start under close supervision and consider a lower starting dose
- Children 20 to <35 kg: 10 mg once daily, maximum 20 mg
- Children ≥35 kg: 20 mg once daily, maximum 40 mg
- Not recommended in children <1 year of age
Contraindications
- Co-administration of aliskiren in patients with diabetes
Important warnings
- Boxed warning: fetal toxicity; discontinue as soon as possible when pregnancy is detected
- Hypotension in volume- or salt-depleted patients
- Changes in renal function up to acute renal failure in susceptible patients
- Sprue-like enteropathy with severe chronic diarrhea and weight loss
- Hyperkalemia; monitor potassium
Side effects
- Events generally mild and transient; dizziness more frequent than with placebo
- Facial edema reported; angioedema reported with angiotensin II antagonists
Interactions
- Potassium-sparing diuretics, potassium supplements, potassium salt substitutes, heparin: increased potassium
- NSAIDs including COX-2 inhibitors: possible deterioration of renal function
- Dual RAS blockade (e.g. aliskiren): avoid, contraindicated in diabetes
- Lithium: increased levels and toxicity; colesevelam: give olmesartan at least 4 hours before
Pregnancy and breastfeeding
- Can cause fetal harm; discontinue as soon as possible when pregnancy is detected
- Breastfeeding: decide whether to discontinue nursing or the drug
