Memantine is an NMDA receptor antagonist used to treat patients with moderate to severe Alzheimer's disease.
What it is used for
- Moderate to severe Alzheimer's disease
Dosing
- Started only if a caregiver regularly monitors intake; reassessment preferably within three months
- Titration by 5 mg per week: week 1 5 mg, week 2 10 mg, week 3 15 mg, from week 4 20 mg daily
- Maintenance dose 20 mg once daily, which is also the maximum daily dose
- Oral solution: 5 mg equals one pump actuation (0.5 ml); 20 mg equals four pump actuations
- Moderate renal impairment (CrCl 30-49 ml/min): 10 mg daily, increased up to 20 mg after at least 7 days if tolerated
- Severe renal impairment (CrCl 5-29 ml/min): 10 mg daily; not recommended in severe hepatic impairment
Contraindications
- Hypersensitivity to the active substance or any excipient
Important warnings
- Caution in epilepsy, history of convulsions or predisposing factors
- Other NMDA antagonists (amantadine, ketamine, dextromethorphan) should be avoided
- Factors raising urine pH (e.g. drastic diet change, renal tubular acidosis, Proteus urinary infections) need monitoring
- Close supervision after recent myocardial infarction, uncompensated heart failure or uncontrolled hypertension
- Oral solution contains sorbitol: not for hereditary fructose intolerance
Side effects
- Common: dizziness, headache, constipation, somnolence, hypertension, dyspnoea, balance disorder, elevated liver function tests
- Uncommon: confusion, hallucinations, vomiting, cardiac failure, venous thrombosis
- Very rare: seizures; isolated cases of psychotic reactions and pancreatitis
Interactions
- Effects of L-dopa, dopaminergic agonists and anticholinergics may be enhanced; barbiturates and neuroleptics may be reduced
- Amantadine should be avoided (risk of pharmacotoxic psychosis); possibly also ketamine and dextromethorphan
- Dantrolene or baclofen: modified effects, dose adjustment may be needed
Pregnancy and breastfeeding
- Not to be used during pregnancy unless clearly necessary
- Women taking memantine should not breast-feed
