Key facts
Maraviroc (Celsentri) is a CCR5 co-receptor antagonist used with other antiretrovirals in treatment-experienced patients infected with only CCR5-tropic HIV-1.
What it is used for
- Celsentri film-coated tablets: in combination with other antiretrovirals for treatment-experienced adults, adolescents and children weighing at least 30 kg infected with only CCR5-tropic HIV-1.
- Celsentri 20 mg/mL oral solution: the same use in treatment-experienced patients from 2 years of age and weighing at least 10 kg.
- Only CCR5-tropic virus must be confirmed with a validated test on a newly drawn blood sample before treatment.
Dosing
- Adults (tablets or oral solution), depending on concomitant medicines: 150 mg twice daily with a potent CYP3A inhibitor (with or without an inducer), 300 mg twice daily without potent CYP3A inhibitors or inducers, 600 mg twice daily with a potent CYP3A inducer without an inhibitor.
- Children and adolescents: dose based on body weight and concomitant medicines, not exceeding the adult dose; see the full text for the weight tables.
- Adults with creatinine clearance <80 mL/min receiving potent CYP3A4 inhibitors: 150 mg once daily; caution in severe renal impairment (CLcr <30 mL/min).
- Limited data in hepatic impairment; see the full text.
Contraindications
- Celsentri film-coated tablets: hypersensitivity to the active substance, to peanut or soya, or to any of the excipients.
- Celsentri oral solution: hypersensitivity to the active substance or to any of the excipients.
Important warnings
- Hepatotoxicity and hepatic failure with allergic features have been reported; discontinuation should be strongly considered with signs of acute hepatitis, especially with suspected hypersensitivity.
- Severe and potentially life-threatening skin and hypersensitivity reactions; stop immediately if they develop.
- Symptomatic postural hypotension, particularly at higher doses, with blood-pressure-lowering medicines or in severe renal insufficiency.
- Coronary heart disease events were more common with maraviroc than placebo in treatment-experienced patients; caution in severe cardiovascular disease.
Side effects
- Common: nausea, diarrhoea, fatigue and headache (the most frequently reported reactions).
- Serious: hepatotoxicity, severe skin and hypersensitivity reactions, postural hypotension.
Interactions
- Metabolised by CYP3A4 and CYP3A5; CYP3A4 inducers decrease and inhibitors increase maraviroc levels, so the dose is adjusted.
- Potent CYP3A4 inhibitors include ritonavir-boosted protease inhibitors (except tipranavir/ritonavir), cobicistat, itraconazole, voriconazole, clarithromycin and telithromycin.
- St John's wort is not recommended (may reduce maraviroc levels and response).
Pregnancy and breastfeeding
- Limited data in pregnant women; maraviroc should be used during pregnancy only if the expected benefit justifies the potential risk to the foetus.
