Key facts

Lacosamide is an antiepileptic medicine. It is used in epilepsy for partial-onset seizures and as add-on therapy for primary generalised tonic-clonic seizures (tablets, syrup, solution for infusion).

What it is used for

  • Monotherapy of partial-onset seizures with or without secondary generalisation in adults, adolescents and children from 2 years
  • Adjunctive therapy of partial-onset seizures from 2 years
  • Adjunctive therapy of primary generalised tonic-clonic seizures in idiopathic generalised epilepsy from 4 years

Dosing

  • Taken twice a day, approximately 12 hours apart
  • 50 kg or more and adults, monotherapy: start 50 mg twice a day, after one week 100 mg twice a day; weekly increases up to 300 mg twice a day
  • 50 kg or more and adults, adjunctive: start 50 mg twice a day, after one week 100 mg twice a day; maximum 200 mg twice a day
  • Alternatively a 200 mg loading dose followed by 100 mg twice a day, under medical supervision
  • Children under 50 kg: start 1 mg/kg twice a day, weight-based maximum doses; syrup preferred
  • Take a missed dose at once unless the next one is due within 6 hours; never a double dose

Contraindications

  • Hypersensitivity to the active substance or any excipient
  • Known second- or third-degree atrioventricular (AV) block

Important warnings

  • Suicidal ideation and behaviour: monitor for signs
  • Dose-related PR prolongation; caution in conduction problems, severe cardiac disease, the elderly and with drugs affecting cardiac conduction
  • After a loading dose, possible higher incidence of serious cardiac arrhythmia and CNS adverse reactions
  • Dizziness may increase the risk of accidental injury or falls

Side effects

  • Most frequent (≥ 10%): dizziness, headache, nausea, diplopia
  • Usually mild to moderate, some dose-related; CNS and GI reactions usually decreased over time
  • Dizziness was the most common reason for discontinuation

Interactions

  • Caution with drugs that prolong the PR interval (including sodium channel blocking antiepileptics) and with antiarrhythmics
  • Generally low interaction potential; CYP2C9, CYP2C19 and CYP3A4 form the O-desmethyl metabolite

Pregnancy and breastfeeding

  • Discuss family planning and contraception with women of childbearing potential; re-evaluate use carefully if pregnancy is planned
