Key facts

Insulin aspart is a rapid-acting insulin analogue. It is used as NovoRapid, as Fiasp (mealtime insulin) and as the premixed NovoMix 30/50 for the treatment of diabetes mellitus.

What it is used for

  • NovoRapid and Fiasp: diabetes mellitus in adults, adolescents and children aged 1 year and above
  • NovoMix 30: diabetes mellitus from 10 years of age
  • NovoMix 50: diabetes mellitus in adults

Dosing

  • Dosing is individual; blood glucose monitoring and dose adjustment are recommended
  • NovoRapid: requirement usually 0.5 to 1.0 unit/kg/day; in basal-bolus regimens 50–70% as NovoRapid, with basal insulin
  • Fiasp: up to 2 minutes before the start of the meal, optionally up to 20 minutes after; type 2: initial dose 4 units at one or more meals
  • NovoMix 30 in type 2: start with 6 units at breakfast and 6 units at dinner, or 12 units once daily at dinner
  • NovoMix 30 and 50 not given intravenously or in insulin pumps
  • In renal or hepatic impairment and in the elderly, intensified monitoring and individual dose adjustment

Contraindications

  • Hypersensitivity to the active substance or any of the excipients

Important warnings

  • Hypoglycaemia with omitted meals, unplanned exercise or too high a dose; warning symptoms may change
  • Fiasp: hypoglycaemia may occur earlier due to the earlier onset of action
  • Hyperglycaemia and diabetic ketoacidosis with inadequate dosing or discontinuation, especially in type 1
  • NovoMix: intravenous use may cause severe hypoglycaemia
  • Seek medical advice before travelling across time zones

Side effects

  • Most frequent: hypoglycaemia
  • At treatment start: refraction anomalies, oedema, injection site reactions
  • Lipodystrophy, cutaneous amyloidosis, allergic skin reactions, rarely anaphylaxis
  • Temporary worsening of diabetic retinopathy with abrupt glucose improvement; acute painful neuropathy

Interactions

  • Reduce insulin requirement: oral antidiabetics, GLP-1 agonists, MAOIs, beta-blockers, ACE inhibitors, salicylates, anabolic steroids, sulphonamides
  • Increase insulin requirement: oral contraceptives, thiazides, glucocorticoids, thyroid hormones, sympathomimetics, growth hormone, danazol
  • Beta-blockers may mask hypoglycaemia; alcohol may intensify or reduce the effect

Pregnancy and breastfeeding

  • NovoRapid and Fiasp can be used in pregnancy; experience with NovoMix is limited
  • Insulin requirements usually fall in the first trimester and rise in the second and third
