Key facts
Glimepiride is, per the US prescribing information, a sulfonylurea used as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
What it is used for
- Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
- Not for type 1 diabetes or diabetic ketoacidosis.
Dosing
- Take with breakfast or the first main meal of the day; starting dose 1 mg or 2 mg once daily.
- Increased risk of hypoglycemia (e.g. elderly, renal impairment): start with 1 mg once daily and titrate conservatively.
- From 2 mg daily, increase in steps of 1 mg or 2 mg based on glycemic response, no more often than every 1 to 2 weeks; maximum 8 mg once daily.
- Switching from longer half-life sulfonylureas (e.g. chlorpropamide): overlapping effect possible for 1 to 2 weeks.
- With colesevelam: take glimepiride at least 4 hours before.
Contraindications
- Hypersensitivity to glimepiride or any of its ingredients.
- History of allergic reaction to sulfonamide derivatives.
Important warnings
- Hypoglycemia: all sulfonylureas can cause severe hypoglycemia, which may impair concentration and reaction (e.g. driving); warning symptoms may be blunted in autonomic neuropathy, the elderly or with beta-blockers.
- Hypersensitivity reactions including anaphylaxis, angioedema and Stevens-Johnson syndrome; discontinue promptly if suspected.
- Hemolytic anemia, particularly in G6PD deficiency.
- Possible increased risk of cardiovascular mortality with sulfonylureas (UGDP study with tolbutamide).
Side effects
- Most common in trials: hypoglycemia, dizziness, asthenia, headache and nausea.
- In placebo-controlled trials included: headache 8.2%, accidental injury 5.8%, flu syndrome 5.4%, nausea 5.0%.
- Postmarketing: impaired liver function up to liver failure, blood count changes, photosensitivity, hyponatremia/SIADH.
Interactions
- Increased glucose lowering with, among others, other antidiabetics, insulin, ACE inhibitors, fluconazole, clarithromycin, quinolones, NSAIDs, salicylates and MAO inhibitors; monitor closely for hypoglycemia.
- Reduced effect with, among others, danazol, glucagon, somatropin, protease inhibitors and atypical antipsychotics.
- Colesevelam reduces glimepiride exposure.
Pregnancy and breastfeeding
- Sulfonylureas cross the placenta; prolonged severe neonatal hypoglycemia has been reported, so stop at least two weeks before expected delivery.
- Monitor breastfed infants for signs of hypoglycemia.
