Key facts

Gabapentin is an antiepileptic drug. Per the US label it is used for postherpetic neuralgia and as adjunctive therapy for partial onset seizures.

What it is used for

  • Postherpetic neuralgia in adults
  • Adjunctive therapy for partial onset seizures, with and without secondary generalization, in adults and pediatric patients 3 years and older with epilepsy

Dosing

  • Postherpetic neuralgia (adults): Day 1 a single 300 mg dose, Day 2 600 mg/day (300 mg twice daily), Day 3 900 mg/day (300 mg three times daily); then titrate as needed up to 1800 mg/day (600 mg three times daily)
  • Epilepsy, 12 years and above: start 300 mg three times a day; maintenance 300 mg to 600 mg three times a day; no more than 12 hours between doses
  • Epilepsy, children 3 to 11 years: start 10 mg/kg/day to 15 mg/kg/day in three divided doses; maintenance 40 mg/kg/day (3 to 4 years) or 25 mg/kg/day to 35 mg/kg/day (5 to 11 years)
  • Renal impairment or hemodialysis (12 years and older): reduced dosing by creatinine clearance per the label table, with a supplemental dose after hemodialysis

Contraindications

  • Hypersensitivity to gabapentin or its ingredients

Important warnings

  • DRESS/multiorgan hypersensitivity, sometimes fatal; evaluate fever, rash or lymphadenopathy immediately and discontinue if no alternative cause is found
  • Anaphylaxis and angioedema
  • Somnolence, sedation and dizziness
  • Suicidal behavior and ideation, as with other antiepileptic drugs
  • Do not stop abruptly (increased seizure frequency, withdrawal symptoms); taper over at least one week
  • Serious, life-threatening respiratory depression with CNS depressants such as opioids or with underlying respiratory impairment

Side effects

  • Postherpetic neuralgia: most common were dizziness, somnolence and peripheral edema
  • Epilepsy trials (over 12 years): somnolence 19% vs 9% on placebo, dizziness 17% vs 7%, ataxia 13% vs 6%
  • Serious: DRESS, anaphylaxis, status epilepticus, respiratory depression; neuropsychiatric reactions in children 3 to 12 years

Interactions

  • Opioids (e.g., morphine, hydrocodone, oxycodone, buprenorphine): respiratory depression and sedation, sometimes fatal
  • Hydrocodone: gabapentin decreases hydrocodone exposure
  • Antacids with aluminum/magnesium hydroxide (Maalox) reduce bioavailability by about 20%; take gabapentin at least 2 hours after
  • False positive urinary protein dipstick readings possible

Pregnancy and breastfeeding

  • Pregnancy: cohort data have not indicated an increased risk of major birth defects or miscarriage, with methodological limitations; developmental toxicity in animals
  • Lactation: gabapentin is secreted in human milk; effects on the breastfed infant are unknown
