Key facts

Fosaprepitant (Ivemend) is an intravenous prodrug of aprepitant, an NK1 receptor antagonist. It is used to prevent nausea and vomiting with chemotherapy.

What it is used for

  • Prevention of nausea and vomiting associated with highly and moderately emetogenic cancer chemotherapy in adults and children aged 6 months and older, as part of combination therapy.

Dosing

  • Adults: 150 mg infused over 20-30 minutes on Day 1, starting approximately 30 minutes before chemotherapy, with a corticosteroid and a 5-HT3 antagonist.
  • Highly emetogenic chemotherapy (adults): dexamethasone 12 mg on Day 1, 8 mg on the second day and 8 mg twice daily on the third and fourth days.
  • Moderately emetogenic chemotherapy (adults): dexamethasone 12 mg on Day 1 only.
  • Children aged 6 months and older (at least 6 kg): age-based regimens, e.g. 12 years and older 115 mg on Day 1, then 80 mg; see the full text.

Contraindications

  • Hypersensitivity to the active substance, polysorbate 80 or any of the other excipients.
  • Co-administration with pimozide, terfenadine, astemizole or cisapride.

Important warnings

  • Immediate hypersensitivity reactions including anaphylaxis during or soon after infusion; do not reinitiate the infusion.
  • Infusion site reactions have been reported.
  • Use with caution in moderate to severe hepatic impairment.
  • Caution with CYP3A4 substrates with a narrow therapeutic range (e.g. ciclosporin, tacrolimus, fentanyl) and with irinotecan.
  • In patients on warfarin, monitor INR closely for 14 days.

Side effects

  • More frequent than with standard therapy (aprepitant, adults): hiccups, ALT increased, dyspepsia, constipation, headache, decreased appetite; fatigue with MEC.
  • In children: hiccups and flushing.

Interactions

  • Weak CYP3A4 inhibitor; may increase levels of CYP3A4 substrates.
  • Hormonal contraceptive efficacy may be reduced during and for 28 days after administration; use non-hormonal back-up contraception for 2 months.
  • The dexamethasone doses in the regimen already account for the interaction.

Pregnancy and breastfeeding

  • No clinical data on exposed pregnancies; reproductive toxicity potential not fully characterised. See the full text.
