Key facts

Aprepitant is a neurokinin-1 receptor antagonist used to prevent nausea and vomiting with highly and moderately emetogenic cancer chemotherapy, as part of combination therapy.

What it is used for

  • Hard capsules: prevention of chemotherapy-induced nausea and vomiting in adults and adolescents from the age of 12
  • Powder for oral suspension: children from 6 months to less than 12 years
  • Always given as part of combination therapy

Dosing

  • Adults (capsules): 125 mg one hour before chemotherapy on Day 1, 80 mg in the morning on Days 2 and 3, with a corticosteroid and a 5-HT3 antagonist
  • Adolescents aged 12 through 17 (capsules): 125 mg on Day 1, 80 mg on Days 2 and 3
  • Oral suspension (6 months to less than 12 years, at least 6 kg): Day 1 3 mg/kg (max 125 mg), Days 2 and 3 2 mg/kg (max 80 mg)
  • In children and adolescents, a co-administered corticosteroid is given at 50 % of the usual dose
  • No dose adjustment in the elderly or renal impairment; caution in moderate to severe hepatic impairment

Contraindications

  • Hypersensitivity to the active substance or any excipient
  • Co-administration with pimozide, terfenadine, astemizole or cisapride

Important warnings

  • Caution in moderate to severe hepatic impairment (limited or no data)
  • Caution with oral CYP3A4 substrates with a narrow therapeutic range (e.g. cyclosporine, tacrolimus, fentanyl); particular caution with irinotecan
  • Warfarin: INR monitored closely during treatment and for 14 days after each 3-day course
  • Hormonal contraceptives may be less effective during and for 28 days after administration

Side effects

  • Adults (HEC): hiccups, ALT increased, dyspepsia, constipation, headache, decreased appetite
  • Adults (MEC): fatigue
  • Paediatric patients: hiccups and flushing

Interactions

  • Aprepitant is a substrate, moderate inhibitor and inducer of CYP3A4, and an inducer of CYP2C9
  • After treatment ends: transient mild induction of CYP2C9, CYP3A4 and glucuronidation
  • The adult dexamethasone doses account for the interaction

Pregnancy and breastfeeding

  • Non-hormonal back-up contraception during treatment and for 2 months after the last dose
