Boxed warning

Antidepressants increased the risk of suicidal thoughts and behavior in children, adolescents, and young adults in short-term studies. These studies did not show an increase in the risk of suicidal thoughts and behavior with antidepressant use in patients over age 24; there was a reduction in risk with antidepressant use in patients aged 65 and older [see Warnings and Precautions (5.1)].

In patients of all ages who are started on antidepressant therapy, monitor closely for worsening, and for emergence of suicidal thoughts and behaviors. Advise families and caregivers of the need for close observation and communication with the prescriber [see Warnings and Precautions (5.1)].

Desvenlafaxine is not approved for use in pediatric patients [see Use in Specific Populations (8.4) ].

Key facts

What it is used for

  • Major depressive disorder (MDD) in adults

Dosing

  • 50 mg once daily, with or without food, at about the same time each day; this is both the starting and the therapeutic dose
  • Doses above 50 mg per day gave no additional benefit and more adverse reactions
  • Swallow tablets whole; do not divide, crush, chew or dissolve
  • Moderate renal impairment: maximum 50 mg per day; severe renal impairment or end-stage renal disease: maximum 25 mg every day or 50 mg every other day; no supplemental doses after dialysis
  • Moderate to severe hepatic impairment: 50 mg per day; escalation above 100 mg per day is not recommended
  • Stop gradually rather than abruptly; for switching to or from an MAOI, see the full text

Contraindications

  • Hypersensitivity to desvenlafaxine, venlafaxine or any excipient (angioedema has been reported)
  • Use with an MAOI for psychiatric disorders or within 7 days of stopping desvenlafaxine; starting desvenlafaxine within 14 days of stopping such an MAOI
  • Starting desvenlafaxine in patients treated with linezolid or intravenous methylene blue

  • Other serotonergic drugs: increased risk of serotonin syndrome
