Key facts

Angiotensin II is a vasopressor (synthetic peptide hormone). It is used in adults with refractory hypotension in septic or other distributive shock.

What it is used for

  • Refractory hypotension in adults with septic or other distributive shock who remain hypotensive despite adequate volume restitution, catecholamines and other available vasopressor therapies

Dosing

  • Hospital use only, by physicians experienced in treating shock; continuous intravenous infusion after dilution, preferably via a central venous line
  • Starting dose: 20 ng/kg per minute
  • May be titrated as often as every 5 minutes in steps of up to 15 ng/kg per minute, according to target MAP (usually 65–75 mmHg)
  • Do not exceed 80 ng/kg per minute during the first 3 hours; maintenance doses not above 40 ng/kg per minute
  • Withdraw gradually in decrements of up to 15 ng/kg per minute to avoid hypotension
  • No special dose adjustment in the elderly or in renal or hepatic impairment; no data in children

Contraindications

  • Hypersensitivity to the active substance or any excipient

Important warnings

  • Clinical experience limited to septic or other distributive shock; not recommended in other shock types (e.g. cardiogenic shock)
  • Thromboembolic events; concurrent VTE prophylaxis unless contraindicated
  • Peripheral ischaemia; use the lowest compatible dose
  • Abrupt withdrawal may cause hypotension or worsening of shock

Side effects

  • Very common: thromboembolic events, transient hypertension
  • Common: tachycardia, peripheral ischaemia
  • Transient hypertension at the starting dose may be promptly mitigated by down-titration

Interactions

  • Recent ACE inhibitors: possibly increased response
  • Recent angiotensin II receptor blockers: possibly reduced response
  • Other vasopressors: additive effect on MAP; their doses may need reduction

Pregnancy and breastfeeding

  • Pregnancy: limited data; avoid if possible and weigh benefit against possible fetal risk
  • Breast-feeding should be discontinued during treatment
