Key facts

Anagrelide is a platelet-reducing agent (phosphodiesterase III inhibitor) used to lower elevated platelet counts in essential thrombocythaemia (Xagrid, hard capsules).

What it is used for

  • Reduction of elevated platelet counts in at-risk essential thrombocythaemia patients who are intolerant to their current therapy or whose platelet counts are not reduced to an acceptable level by it.

Dosing

  • Starting dose 1 mg/day in two divided doses of 0.5 mg, maintained for at least one week.
  • Then titrate individually to the lowest effective dose that keeps platelets below 600 x 10⁹/l (ideally between 150 x 10⁹/l and 400 x 10⁹/l).
  • Dose increments must not exceed 0.5 mg/day in any one week; the maximum single dose should not exceed 2.5 mg.
  • Most patients respond adequately at 1 to 3 mg/day.
  • Elderly: no different regimen. Mild hepatic impairment or renal impairment: assess risks and benefits before starting.

Contraindications

  • Hypersensitivity to anagrelide or any excipient.
  • Moderate or severe hepatic impairment.
  • Moderate or severe renal impairment (creatinine clearance <50 ml/min).

Important warnings

  • Avoid abrupt discontinuation: risk of a sudden increase in platelet counts with potentially fatal thrombotic complications such as cerebral infarction.
  • Serious cardiovascular events (including torsade de pointes, ventricular tachycardia, cardiomyopathy, congestive heart failure); baseline ECG and echocardiography and regular monitoring; caution with risk factors for QT prolongation.
  • Correct hypokalaemia or hypomagnesaemia before starting.
  • Cases of pulmonary hypertension have been reported.
  • Regular monitoring of blood count, liver and renal function and electrolytes; not recommended if transaminases are >5 times the upper limit of normal.
  • Contains lactose.

Side effects

  • Most common: headache (approximately 14%), palpitations (approximately 9%), fluid retention and nausea (each approximately 6%) and diarrhoea (5%).
  • Serious: cardiovascular events such as torsade de pointes, ventricular tachycardia, cardiomyopathy and congestive heart failure, and pulmonary hypertension.
  • Other reactions and frequencies: see the full text.

Interactions

  • CYP1A2 inhibitors (e.g. fluvoxamine, enoxacin) could impair anagrelide clearance; CYP1A2 inducers (e.g. omeprazole) could decrease exposure.
  • Effects of other PDE III inhibitors (e.g. milrinone, enoximone, cilostazol) may be exacerbated.
  • With acetylsalicylic acid, antiplatelet effects may be enhanced; major haemorrhages have occurred.
  • Caution with medicines that prolong the QTc interval.

Pregnancy and breastfeeding

  • Not recommended during pregnancy; women of child-bearing potential should use adequate contraception.
  • Breast-feeding should be discontinued during treatment.
