Key facts
Crisantaspase (Enrylaze) is a recombinant Erwinia chrysanthemi asparaginase. It is used as part of multi-agent chemotherapy for acute lymphoblastic leukaemia and lymphoblastic lymphoma after hypersensitivity or silent inactivation to E. coli-derived asparaginase.
What it is used for
- Acute lymphoblastic leukaemia (ALL) and lymphoblastic lymphoma (LBL) as a component of multi-agent chemotherapy
- Adults and children aged 1 month and older who developed hypersensitivity or silent inactivation to E. coli-derived asparaginase
Dosing
- Every 48 hours: 25 mg/m2 intramuscularly or intravenously
- Alternatively Monday/Wednesday/Friday: 25 mg/m2 on Monday and Wednesday and 50 mg/m2 on Friday (routes as given in the full text)
- With intravenous use, premedication with paracetamol, an H1 and an H2 receptor blocker should be considered
- Trough serum asparaginase activity should be monitored and the schedule adapted if needed
- Only by personnel experienced with antineoplastics, with anaphylaxis treatment available
Contraindications
- History of severe hypersensitivity reactions to the active substance
- Severe pancreatitis or history of severe pancreatitis during previous asparaginase therapy
- Severe thrombosis or severe haemorrhagic events during previous asparaginase therapy
Important warnings
- Grade 3 and 4 hypersensitivity reactions have occurred; may be more frequent with intravenous use
- Pancreatitis, thrombosis and haemorrhage can occur
- Hepatotoxicity: monitor bilirubin and transaminases before and during treatment
- Asparaginase activity varies substantially between patients; level monitoring is required
Side effects
- Most common: anaemia, vomiting, thrombocytopenia, neutropenia, nausea, febrile neutropenia, fatigue, pyrexia
- Also common: decreased appetite, increased transaminases, abdominal pain, headache, diarrhoea
- Most frequent serious reactions: febrile neutropenia, pyrexia, vomiting, sepsis, hypersensitivity, pancreatitis
Interactions
- No interaction studies; asparaginase may increase toxicity of other medicines through effects on liver function
- Vincristine: asparaginase given concurrently or immediately before may increase vincristine toxicity
- An indirect interaction with oral contraceptives cannot be ruled out; non-hormonal contraception should be used
Pregnancy and breastfeeding
- Can cause foetal harm; men and women use effective contraception during and for at least 3 months after treatment
- No breast-feeding during therapy and for two weeks after the last dose
