Key facts
Vericiguat (Verquvo, film-coated tablets) is a soluble guanylate cyclase (sGC) stimulator. It is used for symptomatic chronic heart failure with reduced ejection fraction.
What it is used for
- Adults with symptomatic chronic heart failure with reduced ejection fraction who are stabilised after a recent decompensation event requiring IV therapy; given with other heart failure therapies.
Dosing
- Starting dose 5 mg once daily; double after approximately 2 weeks to the target maintenance dose of 10 mg once daily, as tolerated.
- With symptomatic hypotension within the past 4 weeks: start at 2.5 mg once daily, doubling about every 2 weeks to 10 mg.
- Do not start if SBP <100 mmHg; temporarily down-titrate or stop for symptomatic hypotension or SBP less than 90 mmHg.
- Take with food; not recommended with eGFR <15 mL/min/1.73 m2, dialysis or severe hepatic impairment.
Contraindications
- Hypersensitivity to the active substance or any of the excipients.
- Concomitant use of other sGC stimulators such as riociguat.
Important warnings
- Symptomatic hypotension can occur, particularly with hypovolaemia, severe left ventricular outflow obstruction, autonomic dysfunction, or with antihypertensives or nitrates.
- Combination with PDE5 inhibitors (e.g. sildenafil) is not recommended.
- Not studied with eGFR <15 mL/min/1.73 m2, on dialysis or in severe hepatic impairment.
- Contains lactose.
Side effects
- Very common: hypotension (16.4%).
- Common: anaemia, dizziness, headache, nausea, dyspepsia, vomiting, gastro-oesophageal reflux disease.
Interactions
- Other sGC stimulators (e.g. riociguat) are contraindicated.
- PDE5 inhibitors such as sildenafil add to blood pressure lowering; not recommended.
- Ketoconazole and rifampicin had no clinically meaningful effect on exposure.
Pregnancy and breastfeeding
- As a precaution, should not be used during pregnancy or in women not using contraception.
- Breast-feeding: a risk to the child cannot be excluded.
