Key facts
Ulipristal acetate (ellaOne) is a selective progesterone receptor modulator used for emergency contraception. It inhibits or postpones ovulation and is taken after unprotected intercourse or contraceptive failure.
What it is used for
- Emergency contraception within 120 hours (5 days) of unprotected sexual intercourse or contraceptive failure
- Suitable for any woman of child-bearing age, including adolescents
Dosing
- One 30 mg tablet as soon as possible, no later than 120 hours after the event
- Can be taken at any time during the menstrual cycle, with or without food
- Vomiting within 3 hours: take another tablet
- If the period is late or pregnancy symptoms are present, exclude pregnancy first
- Renal impairment: no dose adjustment; severe hepatic impairment: not recommended
Contraindications
- Hypersensitivity to the active substance or any excipient
Important warnings
- For occasional use only; does not replace a regular contraceptive method
- Not for use in known or suspected pregnancy; does not prevent pregnancy in every case
- If the next period is more than 7 days late or pregnancy is suspected, perform a test; consider ectopic pregnancy
- May reduce the action of hormonal contraception: use a reliable barrier method until the next menstrual period
- Not recommended in severe asthma treated with oral glucocorticoids
Side effects
- Most common: headache, nausea, abdominal pain, dysmenorrhea
- Common also: dizziness, mood disorders, vomiting
- Hypersensitivity reactions including rash, urticaria, angioedema
- Somnolence and blurred vision uncommon; ability to drive may be affected
Interactions
- CYP3A4 inducers (e.g. rifampicin, phenytoin, carbamazepine, barbiturates, St John's wort, efavirenz, nevirapine) markedly reduce levels; not recommended after use in the past 4 weeks, consider a copper IUD
- CYP3A4 inhibitors increase exposure, unlikely to have clinical consequences
- Hormonal contraceptives: their contraceptive action may be reduced; concomitant use with levonorgestrel emergency contraception is not recommended
Pregnancy and breastfeeding
- Pregnancy: not intended for use; does not interrupt an existing pregnancy; limited human data do not suggest a safety concern
- Breast-feeding: excreted in breast milk; breast-feeding not recommended for one week, milk should be expressed and discarded
