Tranexamic acid is an antifibrinolytic. Per the US label, the tablets are used to treat cyclic heavy menstrual bleeding in females of reproductive potential.
What it is used for
- Cyclic heavy menstrual bleeding in females of reproductive potential
Dosing
- Exclude endometrial pathology before prescribing
- Normal renal function: 1,300 mg three times daily (3,900 mg/day) for a maximum of 5 days during menstruation
- Serum creatinine above 1.4 to 2.8 mg/dL: 1,300 mg twice a day; above 2.8 to 5.7 mg/dL: 1,300 mg once a day; above 5.7 mg/dL: 650 mg once a day
- With or without food; swallow tablets whole
Contraindications
- Concomitant combined hormonal contraception
- Active thromboembolic disease or a history of thrombosis or thromboembolism, including retinal vein or artery occlusion
- Intrinsic risk of thrombosis (e.g. thrombogenic valvular or cardiac rhythm disease, hypercoagulopathy)
- Known hypersensitivity to tranexamic acid
Important warnings
- Venous and arterial thrombosis and retinal vessel occlusions reported; with visual or ocular symptoms stop immediately and refer to an ophthalmologist
- Further increased thrombotic risk with combined hormonal contraceptives, particularly in women who are obese or smoke
- Not recommended with factor IX complex or anti-inhibitor coagulant concentrates, or in acute promyelocytic leukemia on tretinoin
Side effects
- In trials: headache (50.4%), nasal and sinus symptoms (25.4%), back pain (20.7%), abdominal pain (19.8%)
- Others: musculoskeletal pain, arthralgia, muscle cramps, migraine, anemia
- Serious: thromboembolic events, hypersensitivity reactions
Interactions
- Combined hormonal contraceptives: contraindicated
- Tissue plasminogen activators: may decrease efficacy of both; discontinue tranexamic acid
- Factor IX complex or anti-inhibitor coagulant concentrates: increased thrombosis risk
Pregnancy and breastfeeding
- Not indicated for use in pregnant women; tranexamic acid crosses the placenta
- Present in breast milk at low concentration
