Boxed warning

For Women with Ductal Carcinoma in Situ (DCIS) and Women at High Risk for Breast Cancer

Serious and life-threatening events associated with tamoxifen in the risk reduction setting (women at high risk for cancer and women with DCIS) include uterine malignancies, stroke and pulmonary embolism. Incidence rates for these events were estimated from the NSABP P-1 trial (see CLINICAL PHARMACOLOGY, Clinical Studies , Reduction in Breast Cancer Incidence in High Risk Women ). Uterine malignancies consist of both endometrial adenocarcinoma (incidence rate per 1,000 women-years of 2.20 for tamoxifen vs. 0.71 for placebo) and uterine sarcoma (incidence rate per 1,000 women-years of 0.17 for tamoxifen vs. 0.4 for placebo)*. For stroke, the incidence rate per 1,000 women-years was 1.43 for tamoxifen vs. 1.00 for placebo**. For pulmonary embolism, the incidence rate per 1,000 women-years was 0.75 for tamoxifen versus 0.25 for placebo**.

Some of the strokes, pulmonary emboli, and uterine malignancies were fatal.

Health care providers should discuss the potential benefits versus the potential risks of these serious events with women at high risk of breast cancer and women with DCIS considering tamoxifen to reduce their risk of developing breast cancer.

The benefits of tamoxifen citrate outweigh its risks in women already diagnosed with breast cancer.

*Updated long-term follow-up data (median length of follow-up is 6.9 years) from NSABP P-1 study. See WARNINGS: Effects on the Uterus-Endometrial Cancer and Uterine Sarcoma .

**See Table 3 under CLINICAL PHARMACOLOGY, Clinical Studies .

Key facts

Tamoxifen is a nonsteroidal agent with antiestrogenic properties. Per the US prescribing information, it is used for metastatic and early breast cancer, DCIS, and risk reduction in women at high risk of breast cancer.

What it is used for

  • Metastatic breast cancer in women and men
  • Adjuvant treatment of node-positive and node-negative breast cancer after surgery and irradiation; reduces contralateral breast cancer
  • DCIS after surgery and radiation, to reduce the risk of invasive breast cancer
  • Reduction of breast cancer incidence in women at high risk

Dosing

  • Breast cancer: 20 to 40 mg daily; doses above 20 mg per day in divided doses (morning and evening)
  • Adjuvant therapy: current data support 5 years of treatment; no indication that doses above 20 mg per day are more effective
  • DCIS and high-risk risk reduction: 20 mg daily for 5 years

Contraindications

  • Known hypersensitivity to tamoxifen or any ingredient
  • Risk reduction and DCIS: concomitant coumarin-type anticoagulants; history of deep-vein thrombosis or pulmonary embolus

Important warnings

  • Boxed warning (DCIS and risk reduction): uterine malignancies, stroke and pulmonary embolism, some fatal; in diagnosed breast cancer the benefits outweigh the risks
  • Endometrial cancer and uterine sarcoma, more frequent with long-term use
  • Hypercalcemia in patients with bone metastases within the first weeks
  • Thromboembolic events; ocular changes (cornea, color vision, retinal vein thrombosis, retinopathy); liver cancer reported

Side effects

  • Generally relatively mild; hot flashes are most frequent
  • Risk reduction trial: hot flashes 64%, vaginal discharge 30%, irregular menses 25%
  • Metastatic disease: increased bone and tumor pain (flare); also edema, pruritus vulvae, depression, dizziness, hair thinning, vaginal dryness

Interactions

  • Coumarin-type anticoagulants: significantly increased effect; monitor prothrombin time
  • Cytotoxic agents: increased risk of thromboembolic events
  • Letrozole concentrations reduced by 37%; rifampin and aminoglutethimide reduce tamoxifen levels

Pregnancy and breastfeeding

  • May cause fetal harm; avoid pregnancy during treatment and for 2 months after, using barrier or nonhormonal contraception
  • Inhibits lactation; breastfeeding not recommended
