Key facts

Sugammadex is a modified gamma-cyclodextrin that binds rocuronium and vecuronium. It is used to reverse neuromuscular blockade induced by these muscle relaxants.

What it is used for

  • Reversal of neuromuscular blockade induced by rocuronium or vecuronium in adults
  • Children and adolescents (birth to 17 years): routine reversal of rocuronium-induced blockade only

Dosing

  • Given only by or under supervision of an anaesthetist; neuromuscular monitoring recommended
  • Routine: 4 mg/kg at 1-2 post-tetanic counts; 2 mg/kg at reappearance of T2
  • Immediate reversal after rocuronium (adults only): 16 mg/kg
  • Recurrence after 2 mg/kg or 4 mg/kg: repeat dose of 4 mg/kg
  • Severe renal impairment (CrCl < 30 mL/min, including dialysis): not recommended
  • Single intravenous bolus injection

Contraindications

  • Hypersensitivity to the active substance or any excipient

Important warnings

  • Ventilatory support until adequate spontaneous respiration; monitor post-operatively for recurrence of blockade
  • Lower than recommended doses increase the risk of recurrence
  • Rare marked bradycardia, occasionally cardiac arrest, within minutes of dosing; monitor haemodynamics
  • Hypersensitivity reactions including anaphylaxis; effects on haemostasis

  • Toremifene and fusidic acid: displacement of rocuronium/vecuronium possible (recurrence of blockade)
  • Hormonal contraceptives: effect may be reduced; a bolus dose equals one missed daily pill

  • No clinical data; caution in pregnant women
  • Breast-feeding: no effect on the infant anticipated after a single dose
