Rivastigmine is a cholinesterase inhibitor. It is used for symptomatic treatment of mild to moderately severe Alzheimer's dementia and (capsules and oral solution) dementia in idiopathic Parkinson's disease.

What it is used for

  • Symptomatic treatment of mild to moderately severe Alzheimer's dementia (capsules, oral solution, patch)
  • Symptomatic treatment of mild to moderately severe dementia in idiopathic Parkinson's disease (capsules and oral solution only)

Dosing

  • Initiated and supervised by an experienced physician; a caregiver should regularly monitor intake
  • Capsules/solution: start 1.5 mg twice a day with meals; increase after at least two weeks per step to 3 mg, 4.5 mg and 6 mg twice a day if well tolerated
  • Capsules/solution: effective dose 3 to 6 mg twice a day; maximum 6 mg twice a day
  • Patch: start 4.6 mg/24 h, increase after at least four weeks to 9.5 mg/24 h (recommended effective dose); 13.3 mg/24 h only under specific conditions
  • Interruption of more than three days: restart at the initial dose
  • Mild to moderate renal or hepatic impairment: no dose adjustment

Contraindications

  • Hypersensitivity to rivastigmine, other carbamate derivatives or any excipient
  • Previous application site reactions suggestive of allergic contact dermatitis with rivastigmine patch

Important warnings

  • Incidence and severity of adverse reactions increase with higher doses
  • Patch: allergic contact dermatitis possible; discontinue treatment in that case
  • Bradycardia and QT prolongation/torsade de pointes; care in sick sinus syndrome and conduction defects
  • Caution in gastric or duodenal ulcers, asthma or obstructive pulmonary disease, urinary obstruction and seizures
  • Patients below 50 kg may experience more adverse reactions; worsening of extrapyramidal symptoms in Parkinson's disease possible

Side effects

  • Most common are gastrointestinal, especially during titration: nausea (38%) and vomiting (23%)
  • Very common: anorexia, dizziness
  • Common: headache, somnolence, tremor, agitation, confusion, anxiety, nightmares
  • Uncommon to rare: syncope, seizures; very rare extrapyramidal symptoms
  • Patch: application site reactions

Interactions

  • May exaggerate the effects of succinylcholine-type muscle relaxants during anaesthesia
  • Should not be given with other cholinomimetics; may interfere with anticholinergics (e.g. oxybutynin, tolterodine)
  • Beta-blockers and other bradycardic agents: caution (bradycardia, syncope)
  • QT-prolonging drugs (e.g. certain antipsychotics, citalopram, IV erythromycin): caution, ECG monitoring may be required

Pregnancy and breastfeeding

  • Pregnancy: no clinical data; should not be used unless clearly necessary
  • Breast-feeding: women on rivastigmine should not breast-feed
