Key facts

Rasagiline is a selective, irreversible MAO-B inhibitor. It is used in adults with idiopathic Parkinson's disease as monotherapy or as adjunct to levodopa.

What it is used for

  • Idiopathic Parkinson's disease in adults as monotherapy (without levodopa)
  • Adjunct to levodopa in patients with end-of-dose fluctuations

Dosing

  • 1 mg (one tablet) once daily, with or without levodopa and with or without food
  • No dose change in the elderly or in renal impairment
  • Mild hepatic impairment: start with caution; moderate: avoid; severe: contraindicated
  • Stop if hepatic impairment progresses from mild to moderate

Contraindications

  • Hypersensitivity to the active substance or any excipient
  • Concomitant other MAO inhibitors (including St John's wort) or pethidine; at least 14 days after stopping rasagiline before starting them
  • Severe hepatic impairment

Important warnings

  • Avoid fluoxetine or fluvoxamine; wait at least five weeks after stopping fluoxetine
  • Dextromethorphan and sympathomimetics (e.g. ephedrine, pseudoephedrine in cold remedies) not recommended
  • With levodopa, increased levodopa adverse reactions, dyskinesia and hypotension may occur
  • Excessive daytime sleepiness and sudden sleep onset episodes

Side effects

  • Monotherapy: headache, depression, vertigo, flu-like symptoms
  • With levodopa: dyskinesia, orthostatic hypotension, fall, abdominal pain, nausea, vomiting, dry mouth
  • Both: musculoskeletal pain (back, neck), arthralgia
  • Hallucinations; skin carcinoma

Interactions

  • Other MAO inhibitors: risk of hypertensive crisis – contraindicated
  • Pethidine: contraindicated
  • Sympathomimetics, dextromethorphan, fluoxetine, fluvoxamine: not recommended or to be avoided

Pregnancy and breastfeeding
