Key facts

Ramucirumab (Cyramza) is a monoclonal antibody against VEGF receptor 2, given as an antiangiogenic infusion for several advanced cancers.

What it is used for

  • Advanced gastric cancer or gastro-oesophageal junction adenocarcinoma after platinum and fluoropyrimidine chemotherapy: with paclitaxel or as monotherapy.
  • Metastatic colorectal cancer after bevacizumab, oxaliplatin and a fluoropyrimidine: with FOLFIRI.
  • Non-small cell lung cancer: with erlotinib first line for activating EGFR mutations; with docetaxel after platinum-based chemotherapy.
  • Advanced or unresectable hepatocellular carcinoma with AFP ≥ 400 ng/ml after sorafenib: monotherapy.

Dosing

  • Gastric cancer with paclitaxel: 8 mg/kg on days 1 and 15 of a 28 day cycle before paclitaxel; as a single agent 8 mg/kg every 2 weeks.
  • Colorectal cancer: 8 mg/kg every 2 weeks before FOLFIRI.
  • NSCLC: with erlotinib 10 mg/kg every two weeks; with docetaxel 10 mg/kg on day 1 of a 21 day cycle.
  • Hepatocellular carcinoma: 8 mg/kg every 2 weeks.
  • Premedication with an H1 antagonist before each infusion; dose reductions for proteinuria, interruption for severe hypertension and for at least 4 weeks before elective surgery (see the full text).

Contraindications

  • Hypersensitivity to the active substance or any excipient.
  • In NSCLC: tumour cavitation or tumour involvement of major vessels.

Important warnings

  • Serious, sometimes fatal arterial thromboembolic events (e.g. myocardial infarction, stroke); permanently discontinue after a severe event.
  • Gastrointestinal perforations and severe bleeding; permanently discontinue after perforation.
  • Infusion-related reactions, mostly with the first infusions; discontinue immediately and permanently for Grade 3 or 4.
  • Severe hypertension: monitor blood pressure before each dose; aneurysms and artery dissections may occur.
  • Impaired wound healing and proteinuria up to nephrotic syndrome.

Side effects

  • Monotherapy, most common: peripheral oedema, hypertension, diarrhoea, abdominal pain, headache, proteinuria and thrombocytopenia.
  • With chemotherapy, most common: fatigue/asthenia, neutropenia, diarrhoea, epistaxis and stomatitis.
  • Most serious: gastrointestinal perforation, severe gastrointestinal haemorrhage, arterial thromboembolic events and posterior reversible encephalopathy syndrome.

Interactions

  • No interactions were observed with paclitaxel, irinotecan, docetaxel or erlotinib.

Pregnancy and breastfeeding

  • Not recommended during pregnancy or in women not using contraception; contraception until 3 months after the last dose.
  • Breast-feeding should be discontinued during treatment and for at least 3 months after the last dose.
