Key facts
Plerixafor (Mozobil) is a CXCR4 chemokine receptor antagonist used with G-CSF to mobilise haematopoietic stem cells into the blood for collection and autologous transplantation.
What it is used for
- Adults with lymphoma or multiple myeloma whose cells mobilise poorly: with G-CSF to enhance stem cell mobilisation for collection and autologous transplantation.
- Children (1 to less than 18 years) with lymphoma or solid malignant tumours: with G-CSF, either pre-emptively when the expected stem cell yield is insufficient, or after a previous failed collection.
Dosing
- Adults: 20 mg fixed dose or 0.24 mg/kg body weight for patients weighing ≤ 83 kg; 0.24 mg/kg for patients weighing > 83 kg; by subcutaneous injection once daily.
- Children: 0.24 mg/kg body weight subcutaneously.
- Given 6 to 11 hours before each apheresis after 4 days of G-CSF pre-treatment; commonly used for 2 to 4 (up to 7) consecutive days.
- The dose should not exceed 40 mg/day.
- Creatinine clearance 20-50 ml/min: reduce to 0.16 mg/kg/day, not exceeding 27 mg/day; no recommendation below 20 ml/min or on haemodialysis.
Contraindications
- Hypersensitivity to the active substance or to any of the excipients.
Important warnings
- Not recommended for stem cell mobilisation in leukaemia, as leukaemic cells may be mobilised and contaminate the apheresis product.
- Increases circulating leukocytes; white blood cell counts should be monitored, and platelet counts checked in all patients undergoing apheresis.
- Allergic reactions, including anaphylactic shock, and vasovagal reactions, orthostatic hypotension or syncope can occur after injection.
- Splenomegaly and splenic rupture have been reported.
Side effects
- Very common: diarrhoea, nausea, injection site reactions.
- Common: insomnia, dizziness, headache, vomiting, abdominal pain, dyspepsia, constipation, flatulence, dry mouth, hyperhidrosis, erythema, arthralgia, musculoskeletal pain, fatigue, malaise.
- Uncommon: allergic reaction, abnormal dreams, nightmares. Not known: splenomegaly, splenic rupture, anaphylactic reactions.
Interactions
- No interaction studies; plerixafor is not metabolised by CYP enzymes and does not inhibit or induce them.
- Adding rituximab did not affect safety or CD34+ cell yield in non-Hodgkin's lymphoma.
Pregnancy and breastfeeding
- Plerixafor is suggested to cause congenital malformations; it should not be used during pregnancy unless clinically required, and women of childbearing potential must use effective contraception.
- Breast-feeding should be discontinued during treatment.
