Key facts

Pazopanib (Votrient, film-coated tablets) is an oral tyrosine kinase inhibitor. It is used for advanced renal cell carcinoma and selected soft-tissue sarcomas.

What it is used for

  • Adults with advanced renal cell carcinoma as first-line treatment or after prior cytokine therapy.
  • Adults with selective subtypes of advanced soft-tissue sarcoma after chemotherapy for metastatic disease or progression within 12 months after (neo)adjuvant therapy.

Dosing

  • Recommended dose: 800 mg once daily; dose changes in 200 mg steps, not exceeding 800 mg.
  • Take without food, at least one hour before or two hours after a meal; do not break or crush tablets.
  • Liver: mild abnormalities 800 mg; moderate hepatic impairment (bilirubin >1.5 to 3 x ULN) 200 mg once daily; not recommended in severe impairment.
  • If a strong CYP3A4 inhibitor cannot be avoided: 400 mg daily.
  • Do not use in children younger than 2 years; caution with creatinine clearance below 30 ml/min.

Contraindications

  • Hypersensitivity to the active substance or any of the excipients.

Important warnings

  • Hepatic failure including fatalities; check liver tests before and during treatment.
  • Hypertension including hypertensive crisis; control blood pressure before starting and check within one week of starting.
  • QT prolongation and torsade de pointes, arterial thrombotic events and haemorrhage, some fatal.
  • Stop treatment at least 7 days before scheduled surgery (wound healing).
  • Hypothyroidism; measure thyroid function at baseline.

Side effects

  • Very common (≥10%): diarrhoea, hair colour change, skin hypopigmentation, exfoliative rash, hypertension, nausea, headache, fatigue, anorexia, vomiting, dysgeusia, stomatitis, weight decreased, elevated transaminases.
  • Serious (<1%): stroke, myocardial ischaemia and infarction, GI perforation and fistula, QT prolongation, haemorrhage; in sarcoma also venous thromboembolism and pneumothorax.

Interactions

  • Strong CYP3A4 inhibitors (e.g. ketoconazole) increase exposure; avoid or reduce the dose.
  • Medicines that raise gastric pH (e.g. esomeprazole) reduce bioavailability by about 40%; if a PPI is needed, take pazopanib in the evening without food together with the PPI.
  • With simvastatin, increased risk of ALT elevations.

Pregnancy and breastfeeding

  • Should not be used during pregnancy unless the woman's clinical condition requires it.
  • Women of childbearing potential: adequate contraception during treatment and for at least 2 weeks after the last dose.
